超纯透析对维持性血液透析患者炎症因子、血生化指标及预后的影响

摘　　要：目的探求超纯透析与常规透析在维持性血液透析患者中的应用效果。方法选择2014年1月至2016年3月在某院血液透析中心行维持性血液透析患者98例,按照抽签法将其随机分为两组,每组各49例。对照组采用常规透析治疗,观察组采用超纯透析治疗。3个月后,比较两组炎性因子水平、血生化指标、不良事件发生率。结果治疗后,观察组白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)及C反应蛋白(CRP)水平均较对照组低,差异具有统计学意义(P<0.05);治疗后,观察组白蛋白(Alb)水平较对照组低,血细胞比容(Hct)、血红蛋白(Hb)水平较对照组高,差异具有统计学意义(P<0.05);观察组总不良事件发生率较对照组低,差异具有统计学意义(P<0.05)。结论相较于常规透析,超纯透析可减轻维持性血液透析治疗患者的机体炎症反应,改善血生化指标,降低不良事件发生率,改善患者预后。Objective To investigate the application effect of ultrapure dialysis and conventional dialysis in the treatment of patients with maintenance hemodialysis（MHD）.Me thods 98 patients with MHD in the hemodialysis center of a hospital from January 2014 to March 2016 were selected and randomly divided into two groups by drawing lots,with 49 cases in each group.The control group were given conventional dialysis while the observation group were treated with ultrapure dialysis.After three months,the inflammatory factors,blood biochemical indexes,incidence of adverse events were compared between the two groups.Re s ults After treatment,the levels of IL-6,TNF-α and CRP of theobservation group were lower than those of the control group and the difference showed statistical significance（P 0.05）.After treatment,the Alb level of the observation group was lower than that of the control group while the Hct and Hb levels were higher than those of the control group（ P 0.05） and the difference showed statistical significance（ P 0.05）.The total incidence of adverse events of the observation group was lower than that of the control group and the difference was of statistical significance（ P 0.05）.Conclus ion Compared with conventional dialysis,ultrapure dialysis for patients with MHD can reduce inflammatory response,improve blood biochemical indexes,decrease the incidence of adverse events and improve the prognosis.
