视黄醇结合蛋白-4与子宫内膜病变的相关性研究

摘　　要：目的研究血清视黄醇结合蛋白-4(RBP-4)及女性生殖激素水平在子宫内膜息肉,子宫内膜增生病变发生、发展及恶变中的临床意义。方法选择2016年1至6月在暨南大学附属宝安妇幼保健院生殖健康科门诊就诊的异常子宫出血或不孕的患者97例,将研究对象以病检结果为标准分为三组,其中子宫内膜增殖期者37例为A组,子宫内膜息肉者39例为B组,子宫内膜增生者21例为C组;采用酶联免疫吸附剂测定法(ELISA)检测三组患者间血清RBP-4水平,化学发光法检测血清女性生殖激素卵泡刺激素(FSH)、黄体生成素(LH)、雌二醇(E2)、孕酮(P)、睾酮(T)、己糖激酶法检测空腹血糖(FBG)、胰岛素水平(INS),比较三组间的差异。结果三组患者的血清FSH、LH、P、T水平比较无显著性差异(F值分别为0.77、0.60、0.28、0.54,均P<0.05),E2水平比较有显著性差异(F=10.52,P<0.01),C组最高,A组最低。三组患者的BMI、RBP-4、腰臀比(WHR)、FBG、INS比较有显著性差异(F值分别为6.37、11.97、4.02、3.97、4.25,均P<0.05),C组最高,A组最低。结论血清RBP-4水平与子宫内膜病变程度呈正相关,可作为子宫内膜病变的预测因子,腹型肥胖、高雌激素、高胰岛素、高血糖水平是子宫内膜息肉和子宫内膜增生发生的高危因素,血清FSH、LH、T、P水平可能与子宫内膜息肉和子宫内膜增生的发生无关。Objective To study the clinical significance of serum retinol binding protein-4（RBP-4）and female reproductive hormone levels in occurrence,development and canceration of endometrial polyps and endometrial hyperplasia.Methods Study subjects were divided into three groups according to pathological results,and 37 cases of endometrial proliferation were in group A,39 cases of endometrial polyps in group B,and 21 cases of endometrial hyperplasia in group C.RBP-4 levels in three groups were detected with enzyme-linked immunosorbent assay（ELISA）,and serum reproductive hormones such as follicle stimulating hormone（FSH）,luteinizing hormone（LH）,estradiol（E2）,progesterone（P）and testosterone（T）were detected with chemiluminescence.Fasting blood glucose（FBG）and insulin levels（INS）were tested with hexokinase method.Differences were compared among three groups.Results There were no significant differences in serum FSH,LH,P and T levels in three groups（Fvalue was 0.77,0.60,0.28 and 0.54,respectively,all P〈0.05）.E2 levels showed significant difference in three groups（F=10.52,P〈0.01）,and that in group C was highest and in group A was lowest.There were significant differences in BMI,RBP-4,WHR,FBG and INS in three groups（F value was 6.37,11.97,4.02,3.97 and 4.25,respectively,all P〈0.05）,and those in group C were highest in group A were lowest.Conclusion Serum RBP-4 level is positively related to degree of endometrial lesion,and it can be used as a predictor for endometrial lesions.Abdominal obesity,high estrogen level,high insulin,high blood glucose are high risk factors for endometrial polyps and endometrial hyperplasia.Serum FSH,LH,T and P may not be related to endometrial polyps and endometrial hyperplasia.
