经支气管镜小容量肺泡灌洗术治疗尘肺的疗效分析

摘　　要：目的研究支气管镜小容量肺泡灌洗术治疗尘肺的临床疗效。方法连续选取本院2014年5月-2016年5月共诊断50例尘肺患者,均采用支气管镜小容量肺泡灌洗术治疗,对比灌洗前后临床症状评分、有效率和不良反应,肺功能和血清炎症因子表达水平。结果灌洗5 d后各临床症状评分较前明显降低,差异均有统计学意义(P<0.05),随访30 d总有效率为94.0%(47/50),无明显不良反应发生。肺功能指标包括第1秒用力呼气容积(FEV1)、用力肺活量(FVC)和每分钟最大通气量(MVV)均较前显著升高,炎症因子白细胞介素-6(IL-6)和肿瘤坏死因子(TNF-α)水平则显著降低,差异均有统计学意义(P<0.05)。结论支气管镜小容量肺泡灌洗术治疗尘肺安全、有效,改善了肺功能,减轻了炎症反应,有较好的应用推广价值。Objective To study the effects of small volume bronchoalveolar lavage with bronchoscopy in treatment of pneumonoconiosis. Methods A total of 50 consecutives as pneumonoconiosis from May 2014 to May 2016 were enrolled and received small volume bronchoalveolar lavage with bronchoscopy, and then the differences of clinical symptom scores,pulmonary function and serum inflammatory factors levels were compared; the effective rate and adverse reactions rate were analyzed. Results The clinical symptom scores 5 d after treatment were all significantly lower than those of before treatment（P〈0.05）. The total efficacy rate was 94.0%（47/50）after 30 d follow-up, and there were no severe complications. After treatment, the pulmonary function including forced expiratory volume in the first second（FEV1）, forced vital capacity（FVC） and maximal ventilatory volume per minute（MVV） were all significantly higher than those of before treatment; what’s more, the levels of interleukin-6（IL-6）and tumor necrosis factor-α（TNF-α）were both significantly lower（P〈0.05）. Conclusion Small volume bronchoalveolar lavage with bronchoscopy for treating pneumonoconiosis was safe and effective, and could improve pulmonary function and reduce inflammation.
