摘　　要：目的探讨"从脾论治"对冠心病稳定型心绞痛脾虚痰浊证患者血同型半胱氨酸的影响及血同型半胱氨酸相关性分析。方法采用多中心、盲法、区组随机、平行对照临床试验设计,选择国内6家医院作为分中心纳入冠心病稳定型心绞痛脾虚痰浊证患者,随机分为益气健脾和血祛痰组、益气健脾化瘀祛痰组和安慰剂对照组,药物干预12周,随访2周。检测心绞痛有效率,血同型半胱氨酸值,血同型半胱氨酸与其他疗效机制指标之间相关系分析,以及安全性指标。结果血同型半胱氨酸与基线变化值历时性分析中,益气健脾和血祛痰组、益气健脾化瘀祛痰组与安慰剂对照组相比,差异有统计学意义(P<0.05);且5-HT、VIP、Ghrelin、NE与血同型半胱氨酸相关系数较高。结论 "从脾论治"可有效降低冠心病患者的血同型半胱氨酸值,并可能干预神经-内分泌-免疫网络。Objective To investigate the effect of ＂ treament according with the spleen ＂ on Hcy in patients with stable angina pectoris and spleen deficiency phlegm syndrome and correlation analysis of Hcy. Methods The clinical trial design was carried out by using multicenter,blind method,and randomized and parallel control group. The eligible patients were randomly assigned to the group of Yi-Qi-Jian-Pi,He-Xue-Qu-Tan and the control group. Treatment lasted for 12 weeks and followed up for 2 weeks. Angina pectoris efficacy and the changes of Hcy were observed. Analysis of the correlation between Hcy and other mechanism indicators. Results In the analysis of Hcy,the changes in value of Yi-Qi-Jian-Pi,He-Xue-Qu-Tan are higher than the changes of. the control group（ P 〈0. 05）. There is significant correlation between Hcy and5-HT、VIP、Ghrelin、NE. Conclusion ＂ Treament According with the Spleen ＂ can effectively reduce Hcy of patients with stable angina pectoris and spleen deficiency phlegm syndrome. This treatment may intervenes in the neuroendocrine-immune network.
