摘　　要：目的分析经皮肝穿刺胆道引流术(PTCD)治疗重症胆管炎及对炎性指标、肝功能与黄疸水平的影响。方法抽取82例于2014年5月至2017年6月入院治疗的重症胆管炎患者,随机分作两组,各41例,施予开腹胆总管切开引流术治疗者作对照组,施予PTCD治疗者作观察组,对比两组炎性指标、肝功能与黄疸水平。结果 (1)研究组降钙素原(PCT)、C-反应蛋白(CRP)、白细胞(WBC)及中性粒细胞比例(NEUT)都比对照组低(P<0.05);(2)研究组丙氨酸氨基转移酶(ALT)、碱性磷酸酶(ALP)、天门冬氨基转移酶(AST)及谷氨酰转肽酶(GGT)比对照组低(P<0.05);(3)研究组总胆红素(TBIL)及直接胆红素(DBil)比对照组低(P<0.05)。结论以PTCD治疗重症胆管炎可改善炎性指标、肝功能及黄疸水平,值得推广。Objective To analysis the effect of percutaneous transhepatic cholangial drainage（PTCD）on acute cholangitis of severe type and its effect on inflammatory markers,liver function and the level of jaundice.Methods 82 patients with acute cholangitis of severe type admitted in our hospital from May2014 to June 2017 were selected as the subjects,two groups based on random method,41 cases in each group,the patients who were treatment with Open choledochal incision and drainage as the control group and treatment with Percutaneous transhepatic cholangial drainage as observation group,the inflammatory markers,liver function and the level of jaundice of two groups were compared.Results After treatmented,（1）the PCT,CRP,NEUT in observation group were lower than that of control group（P〈0.05）;（2）the ALT,ALP,AST and GGT in observation group were lower than that of control group（P〈0.05）;（3）the TBIL and DBil in observation group as well as lower than that of control group（P〈0.05）.ConclusionThe treatment of severe cholangitis with PTCD can improve the inflammatory index,liver function and jaundice,have the value to promotion.
