摘　　要：目的:研究IL-18、TNF-α与原发性高血压病左心室肥厚(LVH)的关系。方法:入选96例原发性高血压病患者及20例健康对照者参与实验,用彩色超声心动仪检测所有受试者心脏腔室各项数值,根据相关数值计算左心室质量指数(LVMI),采用ELISA法检测血清IL-18、TNF-α水平。结果:高血压病患者血清IL-18、TNF-α水平明显高于对照组(P<0.01)。高血压病患者3个级别组间血清IL-18、TNF-α水平两两相比较均具有明显差异(P<0.01);高血压病患者左心室肥厚组IL-18、TNF-α水平明显高于无左心室肥厚组(P<0.01),且分别与左室质量指数呈正相关(r=0.56,P<0.01,r=0.63,P<0.01)。高血压病患者左室肥厚组血清IL-18与TNF-α水平呈正相关(r=0.43,P<0.05)。结论:IL-18、TNF-α可能在原发性高血压病左室肥厚的发病机制中起着重要的作用。Objective： To investigate the relationships between IL-18, TNF-α and LVH in essential hypertension （EH）. Methods： 96 Patients with EH and 20 normal individuals were enrolled in the experiment, The value of left ventricular mass of all participants were evaluated by Doppler echocardiography. The serum concentrations of 1L-18 and TNF-α were measured by ELISA. Results： The serum concentrations of IL-18, TNF-α in hypertensive patients were significantly higher than that in the control group （P〈 0.01）. The levels of IL- 18, TNF-α among the 3 clinical stage were significantly different from each other （P〈 0.01）. The levels of IL- 18, TNF-α were higher in LVH group than that in Non-LVH group （P〈0.01）, and their levels positively correlated with LVMI respectively （r=0.56, P〈0.01 ; r= 0.63, P〈0.01）. The IL-18 level was positively correlated with TNF-α level in the LVH group （r=0.43, P〈0.05）. Conclusion： IL-18, TNF-α probably play an important role in the development and progression of LVH in essential hypertension.
