联动式延续护理在老年糖尿病足Wagner2、3级患者中的应用

摘　　要：目的探讨医院一社区一家庭联动式延续护理对wagner2、3级糖尿病足老年患者的影响。方法选取该院周边对口社区的老年糖尿病患者46例开展医院-社区-家庭联动式延续性护理服务，3个月后观察患者糖尿病足愈合情况及运用中文版糖尿病自我管理行为量表（SDSCA）评价患者自我管理能力。结果46例糖尿病足Wagner2、3级老年患者中，41例患者糖尿病足愈合无复发，3例患者因经济原因放弃治疗，2例患者并发心脑血管疾病死亡；41例患者自我管理能力提高。结论医院一社区一家庭联动式延续护理对促进Wagner2、3级糖尿病足的愈合，降低其复发率，增强医院与社区的合作关系，提高老年糖尿病患者自我管理能力具有积极意义，值得推广应用。Objective To explore the effect of the ＂Hospital-Community-Family＂ coordinated type transitional nursing in elderly patients with diabetic foot Wager 2 and 3 levels. Methods A total of 46 elderly patients with diabetes were given ＂Hospital-Community-Family＂ coordinated type transitional nursing. The healing of diabetic foot was observed and the self-management ability of patients with the Chinese version of diabetes self-management behavior scale （SDSCA） were evaluated after 3 months. Results Of the 46 cases of diabetic foot Wagner 2 and grade 3, 41 patients had no relapse of diabetic foot healing, 3 patients gave up treatment for economic reasons, 2 patients died of cardiovascular and cerebrovascular diseases, and 41 patients improved their self-management ability. Conclusions The ＂Hos- pital-Community-Family＂ coordinated type trasitional nursing is meaningful to promote the healing of Wager 2 and 3 levels of diabetic foot and reduce the recurrence rate. It can enhance the hospital and community relations of cooperation, improve the self-management ability of the elderly patients with diabetes. It is worthy of clinical promotion.
