围绝经期女性心脏瓣膜钙化情况与骨密度的相关性分析

摘　　要：目的:探讨围绝经期女性心脏瓣膜钙化情况与骨密度的相关性。方法:选取2013年1月至2017年1月,我院围绝经期女性患者106例,经超声心动图检查,观察其心脏瓣膜情况,参照钙化性心脏瓣膜病(SCVD)诊断标准,将其分为SCVD组(n=52)和非SCVD组(n=54),所有患者均进行双能X线骨密度测定,对股骨胫、腰椎及全髋关节的骨量进行评估,并分析围绝经期女性心脏瓣膜钙化与骨密度(BMD)的关系。结果:SCVD组髋关节BMD、T值低于非SCVD组,最低T值高于非SCVD组,差异有统计学意义(P<0.05);而两组股骨颈、腰椎BMD、T值比较,差异无统计学意义(P>0.05);经Pearson相关性分析发现,围绝经期女性SCVD与年龄呈正相关(P<0.05),与BMD、最低T值呈负相关(P<0.05)。结论:围绝经期女性心脏瓣膜钙化与BMD有关,推测低BMD可能是围绝经期女性钙化性心脏瓣膜病的危险因素。Objective： To investigate the correlation between cardiac valve calcification and bone mineral density in perimenopausal women. Methods： A total of 106 women with perimenopausal who were enrolled in our hospital from January 2013 to January 2017 were selected as the subject. The heart valve of all cases were observed by echocardiography,and all patients were divided into SCVD group（ n = 52） and non-SCVD group（ n = 54） with reference to the diagnostic criteria of calcific valvular heart disease. All patients underwent dual energy X-ray absorptiometry,and the bone mass of femur tibia,lumbar spine and total hip were evaluated.The relationship between calcification and BMD in perimenopausal women was analyzed. Results： The BMD and T values of hip joint in SCVD group were lower than those in non-SCVD group,the lowest T value was higher than that of non-SCVD group,the difference was statistically significant（ P 〈 0. 05）; But there was no significant difference in BMD and T value of femoral neck and lumbar vertebrae between the two groups（ P 0. 05）. Pearson correlation analysis showed that SCVD in perimenopausal women was positively correlated with age（ P 〈 0. 05）,and negatively correlated with BMD and lowest T value（ P 〈 0. 05）. Conclusion： Cardiac valve calcification in perimenopausal women is associated with BMD,and it is suggested that low BMD may be an independent risk factor for calcific valvular heart disease in perimenopausal women.
