中西医联合卡介苗素治疗慢性荨麻疹的临床疗效及其对患者免疫功能的影响

摘　　要：目的探讨中西医联合卡介苗素治疗慢性荨麻疹的临床疗效。方法选取深圳市宝安区中心医院皮肤科2015年1月—2017年2月收治的慢性荨麻疹患者240例,随机分为对照组与治疗组,各120例。对照组患者采用常规药物治疗,治疗组患者在对照组基础上给予中西医联合卡介苗素治疗,两组患者均持续治疗6周。比较两组患者的临床疗效、治疗前后外周血T淋巴细胞亚群(CD3+细胞分数、CD_4^+细胞分数、CD_8^+细胞分数和CD_4^+/CD_8^+细胞比值)及复发率。结果治疗组患者治疗总有效率高于对照组(P<0.05)。治疗前,两组患者CD3+细胞分数、CD_4^+细胞分数、CD_8^+细胞分数和CD_4^+/CD_8^+细胞比值比较,差异无统计学意义(P>0.05);治疗后,治疗组患者CD3+细胞分数、CD_4^+细胞分数和CD_4^+/CD_8^+细胞比值高于对照组,CD_8^+细胞分数低于对照组(P<0.05)。治疗组患者疾病复发率低于对照组(P<0.05)。结论采用中西医联合卡介苗素治疗慢性荨麻疹的临床疗效确切,其可有效改善患者的临床症状,提高机体免疫功能,降低复发率。Objective To investigate the clinical effect of Chinese-western medicine combined with BCG in treating chronic urticaria and the impact on immune function of patients.Methods A total of 240 cases of chronic urticaria patients were selected from January 2015 to February 2017 in Shenzhen Baoan District Central Hospital,which were randomly divided into control group and treatment group,120 cases in each group.The control group was given routine treatment,the treatment group was given Chinese-western medicine combined with BCG,both groups were treated for 6 weeks.The clinical effect,peripheral blood T cell subsets（CD3＋ cell fraction,CD_4^＋ cell fraction,CD_8^＋ cell fraction and CD_4^＋/CD_8^＋ cell ratio） before and after treatment,recurrence rate were compared between the two groups.Results The total effective rate in treatment group was higher than control group（P0.05）.Before treatment,no significant differences of CD3＋ cell fraction,CD_4^＋ cell fraction,CD_8^＋ cell fraction or CD_4^＋/CD_8^＋ cell ratio was found between the two groups（P0.05）;after treatment,the treatment group of CD3＋ cell fraction,CD_4^＋ cell fraction and CD_4^＋/CD_8^＋ cell ratio were higher than control group,CD_8^＋ cell fraction was lower than control group（P0.05）.Conclusion Chinese-western medicine combined with BCG have an obviously clinical effect in treating chronic urticaria,it can effectively improve the clinical symptoms,and promote immune function of patients,reduce recurrence rate.
