加味温胆汤辅助治疗高血压疗效及对VEGF、Hcy、心肌重构的影响

摘　　要：目的：探讨加味温胆汤辅助治疗高血压的疗效及对血管内皮生长因子（VEGF）、同型半胱氨酸（Hey）及心肌重构的影响。方法：回顾性选取2013年8月_2016年2月河南中医药大学第一附属医院收治的高血压患者138例，根据治疗方法不同分为对照组和观察组。对照组患者采用常规药物治疗，观察组患者在对照组患者治疗基础上用加味温胆汤辅助治疗。观察并比较两组患者血压水平、中医症状积分变化情况、VEGF、Hcy及心机重构指标的水平变化。结果：两组患者治疗后收缩压、舒张压和平均脉压差均较治疗前下降，观察组患者治疗后收缩压、舒张压和平均脉压差均低于对照组患者治疗后（P〈0．05）。两组患者治疗后眩晕、头痛、头如裹、胸闷、呕吐痰涎、食少多寐及失眠积分均较治疗前下降，观察组患者治疗后眩晕、头痛、头如裹、胸闷、呕吐痰涎、食少多寐及失眠积分均低于对照组患者治疗后（P〈0．05）。两组患者治疗后血清VEGF和Hcy水平均较治疗前下降，观察组患者治疗后血清VEGF和Hcy水平均低于对照组患者治疗后（P〈0．05）。两组患者治疗后LVDD水平均较治疗前下降，VTId和VTIs均较治疗前上升（P〈0．05）。观察组患者治疗后LVDD水平低于对照组患者治疗后，VTId和VTIs水平均高于对照组患者治疗后（P〈0．05）。结论：加味温胆汤辅助治疗高血压可有效控制血压，减轻l临床症状，降低血清VEGF和Hey水平，改善血管内皮功能，减少高血压心肌重构。Objective ： To investigate the effect of Jiawei Wendan Decoction on hypertension and its effects on vascular endothelial growth factor（ VEGF）, homocysteine （Hcy） and myocardial remodeling. Methods ： Totally 138 cases of hyper- tension patients in our hospital from August 2013 to February 2016 were retrospectively selected. They were divided into control group and observation group according to different treatment methods. Patients in the control group were treated with conventional medicine, and the observation group was treated with modified Wendan Decoction on the basis of the treatment of the control group. The blood pressure levels, symptom scores, changes of VEGF, Hcy and remodeling inde- xes were observed and compared between the two groups. Results：The systolic pressure, diastolic pressure and mean pulse pressure difference between the two groups were lower than those before treatment. The systolic pressure, diastolic pressure and mean pulse pressure difference in the observation group were lower than those in the control group after treat- ment（P 〈0. 05）. Two groups＇ vertigo, headache, chest tightness, head wrap, vomiting phlegm, eat less and insomnia scores after treatment were lower than those before treatment. After treatment in the observation group, dizziness, head- ache, chest tightness, head wrap, vomiting phlegm, eat less and insomnia score were lower than those of the control group（ P 〈 O. 05）. The serum levels of VEGF and I-Icy were lower in the two groups than those before treatment. The ser- um levels of VEGF and Hcy in the observation group were lower than those in the control group after treatment （ P 〈 O. 05 ）.
