加味海藻玉壶汤联合火针加刺络拨罐治疗囊肿型痤疮的临床观察

摘　　要：目的探讨加味海藻玉壶汤联合火针加刺络拨罐治疗囊肿型痤疮的临床疗效。方法随机将74例囊肿型痤疮患者分为汤剂组)和三联组各37例,汤剂组给予加味海藻玉壶汤治疗,三联组在以上治疗基础上给予火针加刺络拨罐治疗,观察两组疗效。结果两组治疗后的皮损评分及中医症状积分较治疗前下降趋势明显(P<0.05),三联组改善效果较汤剂组更显著(P<0.05);三联组和汤剂组的中医症候总有效率分别为91.89%和72.97%(P<0.05);与治疗前对比,血清IL-2、IL-4水平有显著下降趋势(P<0.05),三联组下降幅度较汤剂组更显著(P<0.05);观察期间,两组均未出现严重不良反应;随访期间,三联组和汤剂组复发率分别为(5.00%和27.27%,P<0.05)。结论加味海藻玉壶汤联合火针加刺络拨罐治疗囊肿型痤疮疗效显著,可有效改善患者炎症反应,降低痤疮复发风险,临床应用安全性高。Objective To explore the clinical efficacy of Jiawei Haizao Yuhu decoction combined with fire needling plus blood-letting puncturing and cupping in the treatment of cystic acne. Methods 74 cases of patients with cystic acne were randomly divided into a decoction group（ n = 37） and a triplet group（ n = 37）. The decoction group was given Jiawei Haizao Yuhu decoction,and the triplet group was given fire needling plus blood-letting puncturing and cupping on this basis,and the efficacy was observed in the two groups. Results The skin lesion score and TCM symptoms scores of two groups after treatment were significantly lower than those before treatment（P〈0. 05）,and the improvement effect of the triplet group was more significant than that of the decoction group（P〈0. 05）. The total effective rates of TCM symptoms were 91.89% and 72. 97% respectively in the triplet group and the decoction group（P〈0. 05）. The levels of serum IL-2 and IL-4 decreased significantly compared with those before treatment（P〈0. 05）,and the decrease in the triplet group was more significant than that in the decoction group（P〈0. 05）. During the observation period,there was no serious adverse effect in the two groups. During the follow-up,the recurrence rates were 5. 00% and 27. 27% respectively in the triplet group and the decoction group（P〈0. 05）. Conclusion Jiawei Haizao Yuhu decoction combined with fire needling plus blood-letting puncturing and cupping can have significant efficacy in treatment of cystic acne. It can effectively improve inflammatory reaction,and reduce the risk of acne recurrence,and thus has high safety in clinical application.
