纵隔型肺癌的X线及CT表现分析

摘　　要：目的通过分析研究纵膈型肺癌患者的X线及CT表现诊断的临床表现,以期为临床诊断纵膈型肺癌提供参考。方法对2013年04月—2018年04月收治的19例经病理诊断或手术确诊的纵膈型肺癌患者的X线片和CT表现进行回顾性分析,归纳总结纵膈型肺癌患者的X线和CT表现。结果本次研究的19例纵膈型肺癌患者中10例为中央型肺癌,5例为周围型肺癌,4例为隐匿性肺癌。纵膈型肺癌主要以中央型肺癌为主,患者表现为单侧上中纵膈增宽,肺门区软组织密度肿块不规则,且肺段支气管存在狭窄或者闭塞情况;6例患者表现为肺不张。周围型肺癌患者表现为上纵膈增宽,纵膈旁肿块病灶内缘紧贴纵膈的胸膜,肿块和肺组织分界不清楚。结论肺癌占位性病变过程中,患者的X线片结果和CT表现各不相同,在临床诊断过程中通过综合观察患者X线表现和CT表现有助于临床诊断。Objective To provide a reference for the clinical diagnosis of mediastinal lung cancer by analysis of Xray and CT findings of patients with mediastinal lung cancer. Methods We retrospectively analyzed the X-ray and CT findings of 19 patients with mediastinal lung cancer diagnosed by pathological diagnosis or surgery from April 2010 to April 2018 in our hospital,and summarized the X-ray and CT findings of patients with mediastinal lung cancer. Results Of the 19 patients with mediastinal lung cancer in this study,10 were with central lung cancer,5 with peripheral lung cancer,and 4 with occult lung cancer. The mediastinal lung cancer is mainly composed of central lung cancer. The patient＇s manifestation is unilateral upper mediastinum widening,irregular soft tissue density in the hilar region,and stenosis or occlusion in the pulmonary bronchus; 6 patients show lung abscess. Peripheral lung cancer patients showed a widening of the mediastinum,and the medial edge of the mediastinal mass was close to the mediastinal pleura. The boundary between the mass and the lung tissue was unclear. Conclusion In the process of space-occupying lesions of lung cancer,the clinical diagnosis of patients with mediastinal lung cancer can be facilitated through careful analysis of the imaging findings of the patient and the clinical symptoms of the patient.
