妇瘤科门诊患者中纤毛菌的检出情况及其临床意义

摘　　要：目的:分析因阴道不适而就诊于妇瘤科门诊的患者中纤毛菌的检出情况,探讨其临床意义。方法:选取2016年4月至2017年3月因"外阴瘙痒、阴道分泌物增多、有异味"等症状在安徽省肿瘤医院(安徽省立医院西区)妇瘤科门诊就诊的672例患者为研究对象,采集阴道分泌物检测纤毛菌及阴道微生态状况。结果:672例患者中,阴道微生态正常者49例(7.3%),异常者623例(92.7%)。纤毛菌感染共71例(10.6%),包含AV组6例、BV组47例、AV合并BV组18例,患者平均年龄45岁。纤毛菌感染多表现出脓细胞增加,与未感染纤毛菌患者相比,其差异具有统计学意义(P<0.05)。结论:纤毛菌感染通常伴有其他致病菌存在,表现出严重的炎症反应。Objective： To investigate the prevalence of sneathia in gynecologic oncology outpatients with vaginal discomfort,and discuss its clinical significance. Methods： 672 gynecologic oncology outpatients with vaginal discomfort（ such as vulvar pruritus,vaginal discharge increased in amount or abnormal in odor） in Department of Gynecologic Oncology,Anhui Provincial Cancer Hospital（ The Western Branch of Anhui Provincial Hospital）,from April 2016 to March 2017 were selected. The presence of sneathia and the vaginal microecology were checked with all selected samples. Results： Among all the 672 cases,49 cases（ 7. 3%） showed balanced vaginal microecology while 623 cases（ 92. 7%） were unbalanced. A total of 71 cases（ 10. 6%） of sneathia infections were detected,including 6 cases of aerobic vaginitis（ AV）,47 cases of bacterial vaginosis（ BV）,and 18 cases co-infected with AV and BV. The mean age of patients with sneathia infection was 45. Besides,sneathia infection often indicated a significantly increased amount of toxic leucocytes（ P 〈 0. 05） compared with uninfected controls. Conclusions：Sneathia infection is often accompanied by the presence of other vaginal pathogens and characterized by serious inflammation.
