摘  要:目的探索CT灌注成像在节拍化放疗对非小细胞肺癌治疗效果评价中的应用价值。方法方便选取2015年8月—2016年8月在该院治疗的57例非小细胞肺癌患者,随机分为治疗组和对照组。治疗组以紫杉醇8 mg/m2+卡铂AUC=0.25 mg/(m L·min)(每周2次)的节拍化疗联合放疗;对照组以紫杉醇45~50 mg/㎡+卡铂AUC=2 mg/(m L·min)(每周1次)与放疗行同步放化疗。治疗前后行CT扫描及血流灌注分析,并评价其参数变化。结果近期疗效分析,治疗组与对照组总有效率分别为86.20%、71.43%(χ~2=2.397,P=0.524),两组总有效率比较差异无统计学意义(P>0.05)。两组患者经治疗后BF(t=2.544,P=0.058)、BV(t=2.205,P=0.001)、PS(t=2.079,P=0.002)测值减小,MTT(t=2.509,P=0.389)增加,血流灌注得到改善,但两组改善程度不同,节拍化放疗组稍显著,其中治疗后BV(P=0.001)、PS(P=0.002)改善差异有统计学意义(P<0.05)。结论 CT灌注成像能够反映非小细胞肺癌患者化疗早期血流变化情况,对肺癌进行早期疗效判断;节拍化放疗为肺癌个体化治疗提供了一种新思路。Objective This paper tries to explore the value of CT perfusion imaging in the evaluation of therapeutic effects of metronomic radiation therapy on non-small cell lung cancer. Methods 57 patients with non-small cell lung cancer treated in this hospital from August 2015 to August 2016 were rconvenient divided into treatment group and control group. The treatment group received paternal radiotherapy combined with paclitaxel 8 mg/m^2+ carboplatin AUC = 0.25 mg/mL.min(twice weekly); the control group received paclitaxel 45-50 mg/m^2+ carboplatin AUC = 2 mg/mL.min(once weekly) concurrent chemotherapy with radiotherapy. Before and after treatment, CT scan and blood perfusion analysis were performed, and their parameter changes were evaluated. Results In the short-term efficacy analysis, the total effective rates of the treatment group and the control group were 86.20% and 71.43%, respectively(χ^2=2.397, P=0.524). There was no significant difference in the total effective rate between the two groups(P〉0.05). After treatment, the values of BF(t=2.544, P=0.058), BV(t=2.205, P=0.001), PS(t=2.079, P=0.002) decreased, and MTT(t=2.509, P=0.389) increased, blood perfusion was improved,but the degree of improvement in the two groups was different, and beat radiotherapy group was slightly significant, among which the improvement of BV(P =0.001) and PS(P =0.002) was statistically significant(P〈0.05). Conclusion CT perfusion imaging can reflect the changes of blood flow in patients with non-small cell lung cancer at the early stage of chemotherapy, and it can be used to judge the early effect of lung cancer. Beat beat radiotherapy provides a new idea for individualized treatment of lung cancer.
