贵州省优质医疗资源下沉基层现状调查研究

摘　　要：目的了解贵州省三级医院医务人员去基层医疗机构服务的意愿并分析贵州省优质医疗资源下沉基层的现状。方法采取随机抽样的方法对贵州省9家三级医院医务人员进行问卷调查;采用Epi Data 3.1将数据双录入,采用SPSS20.0及Excel 2010进行统计描述和χ2检验。结果通过对9家三级医院260名医务人员进行问卷调查,了解医务人员基本情况、医务人员对优质医疗资源下沉制度的了解度、对优质医疗资源下沉基层的意愿以及更接受的下沉方式等,分析贵州省优质医疗资源下沉基层的现状。结论贵州省优质医疗资源下沉基层是可行的;贵州省优质医疗资源下沉基层存在以下主要问题:医务人员对相关政策不够了解;少数医务人员不支持去基层服务且不愿将病人向下转诊;医务人员对医连体和远程医疗的接受度差;三级医院医务人员执业模式受到限制等。并从政策解读、财政扶持、资源整合及人员激励等方面提出解决问题的对策建议。Objective To understand the hospital staffs＇ willingness work at grassroots of the tertiary hospitals and to analyze the current situation of Guizhou＇s high-quality medical resources go down to grassroots. Methods Using the simple random sampling method to choose the tertiary hospitals from 9 major cities in Guizhou Province,and using structural questionnaire to survey medical staffs;using Epi Data 3.1 to do double input and doing statistical description and Chi square test used by SPSS 22.0 and Excel 2010. Results Through conducting self-designed questionnaire survey to the 260 medical staffs of 9 tertiary hospitals,the basic situation of medical staff,the understanding of the medical personnel＇s sinking system of high quality medical resources,the willingness to sink the basic level of high quality medical resources and a more acceptable way to sink in the medical resources are understood. The current situation of high quality medical resources sinking in Guizhou Province is analyzed. Conclusion It is feasible to sink the base of high quality medical resources in Guizhou Province. The survey found that there are some problems,such as the lack of medical staff＇s understanding of relevant policies,the support of some medical staff for basic services,that a small number of medical staff do not support basic services and are unwilling to turn the patients down. Medical staff have poor acceptance of medical linkage and telemedicine,and the practice mode of three level hospitals is limited. The policy interpretation,specific measurements and suggestions should be taken to solve the problem from financial support,resource integration and staff motivation and other aspects.
