摘　　要：目的探讨儿童胚胎发育不良性神经上皮瘤(DNET)的MR特点及鉴别诊断方法 ,以期进一步提高对本病的认识。方法回顾性分析经手术病理证实的15例DNET患儿的临床资料,所选患儿术前均行MR扫描,4例行核磁共振波谱(MRS)分析。结果 15例病灶均为单发,颞叶为主,形状各异;MR平扫以T_1WI低、T_2WI高信号为主,DWI呈略低或低信号。T2-FLAIR序列6例呈中心低、周围高信号的"环形征",5例呈"倒三角征"。增强扫描病灶无强化12例,轻度不均匀强化3例。病灶边界较为清晰,均无占位效应及瘤周水肿。MRS表现为NAA峰均略下降,3例Cho峰大致正常,1例略升高。病变区与正常对照区对比,差异无统计学意义(P>0.05)。结论 DNET好发于儿童及青少年,其MR表现具有一定特征性,熟悉掌握这些特点并做好相似疾病的鉴别诊断,可为术前诊断提供可靠的影像依据。Objective To study MRI features and differential diagnosis of dysembryoplastic neuropithelial tumor（DNET）, in order to increase the understanding of the disease. Methods The clinical data of 15 cases with DNET children confirmed by surgical pathology were analyzed retrospectively, and the children were selected for the preop-erative MR scan and 4 routines（MRS） analysis. Results All focus of 15 cases were single, mainly in the temporal lobe, with different shapes; MRI scan was based on T_1WI low signal and T_2WI high signal, and diffuse weighting showed slightly low or low signal. Six cases by T2-FLAIR were presented in the center with a low signal and the surrounding high signal ＂ring＂, and 5 cases were ＂inverted triangle＂. The enhanced scanning lesion was not enhanced in 12 cases, 3 cases of mild nonuniform enhancement, as no local mass effect and peripheral edema. MRS showed a slight decline in NAA peak, 3 cases of Cho peak were normal, 1 case was slightly elevated. In cmparison to normal control area, the difference was not statistically significant（P0.05）. Conclusion DNET have a high incidence in children and adolescents, and its MRI performance has a certain characteristic, be familiar to master these features and differ-ential diagnosis of the disease, it can provide reliable imaging evidence for preoperative diagnosis of the disease.
