摘  要:目的探索我国职业接触二甲基甲酰胺(DMF)工人血液N-甲基氨甲酰血红蛋白加合物(NMHb)的生物接触限值。方法将某皮革生产企业168名DMF接触工人和30名不接触DMF的行政人员分别纳入同质暴露组和对照组,采用个体采样器采集空气并检测DMF浓度,采集研究对象血液检测NMHb浓度,并进行职业健康检查。比较两组肝功能异常率,分析DMF暴露浓度与血液NMHb浓度的相关性,提出我国NMHb生物接触限值。结果该皮革企业DMF暴露浓度为0.100~648 mg/m3,同质暴露组研究对象血液NMHb浓度为2.38~761 nmol/g Hb。同质暴露组肝功能异常检出率为50.60%,高于对照组的3.33%(P<0.001)。血液NMHb浓度与DMF暴露浓度呈对数线性关系,回归方程为lg(NMHb)=0.552 lg(DMF)+1.319(r=0.862,P<0.01);参考美国政府工业卫生医师协会DMF时间加权浓度30 mg/m3推算得NMHb为136.3 nmol/g Hb。低内暴露组(NMHb≤135 nmol/g Hb)肝功能异常检出率为42.39%,低于高内暴露组(NMHb>135 nmol/g Hb)的60.53%(P<0.05)。在DMF暴露浓度<20 mg/m3组中,93.5%职业接触劳动者血液NMHb浓度<135 nmol/g Hb。结论 DMF暴露与肝功能异常和血液NMHb浓度升高相关,建议我国NMHb生物接触限值设置为135 nmol/g Hb。Objective To study the threshold limit value of N-methylcarbamoylated haemoglobin(NMHb) in the workers' blood exposed to N, N-dimethylformamide(DMF) in China. Methods The 168 workers and 30 administrative personals of the leather factory were chosen as the objects and the controls respectively. The ambient air measurements by personal air sampling at the factory were determined by gas chromatography. All the blood samples of the objects were collected and the concentrations of hemoglobin adducts in blood were determined by gas chromatography-mass spectrometry(GC/MS). The objects all had occupational health examined. Through comparing the abnormal rate of liver function in the two groups,the correlation regression analysis of the DMF exposure concentration and the NMHb concentration in blood, the threshold limit value of NMHb occupational exposed to DMF was proposed. Results The DMF ambient concentrations of the leather factory were 0.100-648 mg/m^3,and the NMHb concentrations of the homogeneity exposure group were 2.38-761 nmol/g Hb. The abnormal rate of liver function in homogeneous exposure group was 50.60%, which was higher than the control group(P〈0.001). The regression equation of the concentrations of NMHb in blood and the concentrations of DMF in the air was lg(NMHb) =0.552 lg(DMF) +1.319(r=0.862,P0.01). On the basis of the threshold limit value of DMF(30 mg/m^3,ACGIH) and the regression equation,the concentrations of NMHb were calculated for 136.3 nmol/g Hb. The abnormal rate of liver function in low internal exposure group(NMHb ≤135 nmol/g Hb) was 42.39%,lower than 60.53% in high internal exposure group(NMHb135 nmol/g Hb)(P0.05). Intheexposuregroup(DMF20 mg/m^3),93.5%ofthe NMHbconcentrationoftheoccupationalexposureworkerswasless than 135 nmol/g Hb. Conclusion The exposure concentration of DMF is correlated with abnormal liver function and the increasing NMHb concentration,and the threshold limit value of NMHb in the blood is recommended to be 135 nm
