右位主动脉弓21例超声分析

摘　　要：目的:探讨右位主动脉弓探查切面及超声表现特点,提高右位主动脉弓的检出率。方法:选择右位主动脉弓患者21例,分析其临床及超声资料。结果:21例右位主动脉弓患者,3例合并心内畸形,2例合并血管畸形,1例合并单脐动脉,2例合并羊水多,11例单纯右位主动脉弓,2例双主动脉弓;18例右位主动脉弓左动脉导管弓,1例右位主动脉弓右位导管弓;5例右位主动脉弓右侧降主动脉,14例右位主动脉弓左侧降主动脉。21例胎儿2例引产,19例胎儿生后均未有气管压迫症状。胎儿生产后行心脏超声检查,17证实右位主动脉弓,2例双主动脉弓。结论:上纵隔三血管气管切面是诊断右位主动脉弓的主要切面,右位主动脉弓在上纵隔三血管气管切面有典型超声特征。单纯右位主动脉弓左位导管弓最常见,右位主动脉弓右位导管弓少见。右位主动脉弓可单独出现,也可以合并其他畸形。Objective：To explore the characteristics of the sections and ultrasonographic features of right aortic arch,and to improve the detection rate of right aortic arch.Methods：21 patients with right aortic arch were selected,and clinical and ultrasound data were retrospectively analyzed.Results：In the 21 cases of right aortic arch,there were 3 cases with cardiac malformation,2 cases with vascular malformations,1 case with single umbilical artery and 2 cases with amniotic fluid,there were 11 cases of simple right aortic arch and 2 cases of double aortic arch.There were 18 cases of left ductus arteriosus of right aortic arch and 1 case of right catheter arch of right aortic arch.There were 5 cases of right descending aorta of right aortic arch and 14 cases of left descending aorta of right aortic arch.In 21 fetuses,2 fetuses were induced,no symptoms of tracheal compression were found in 19 cases of fetuses.The fetus was examined by echocardiography,there were 17 cases of right aortic arch and 2 cases of double aortic arch.Conclusion：The upper mediastinum three vascular trachea section was the main section for the diagnosis of the right aortic arch,there were typical ultrasonic characteristics of right aortic arch at the upper mediastinum three vascular trachea section.The simple left catheter arch of right aortic arch was the most common,the right catheter arch of right aortic arch was rare.Right aortic arch can appear individually and can also merge with other deformities.
