重症手足口病患儿血清Th17、Treg动态变化及预后评估价值

摘　　要：目的:动态监测肠道病毒71型(EV71)阳性重症手足口病患儿外周血Th17、CD4^+CD25^+Treg表达变化及其意义。方法:收治重症手足口病患儿60例作为重症组,同期选择普通组105例,健康对照组30例。监测各组CD4^+CD25^+Treg、Th17及细胞因子TGF-β、IL-6、IL-10、IL-17的表达。结果:重症组Th17、IL-6、IL-10、IL-17的表达明显高于健康对照组(P<0.01),而CD4^+CD25^+Treg、TGF-β的表达明显低于健康对照组(P<0.01)。结论:重症组手足口病患儿中Th17表达升高、CD4^+CD25^+Treg表达降低,且与病情严重程度相关,可作为预测病情严重程度及预后评估的重要参考指标。Objective：To dynamically monitor the expression and significance of Th17 and CD4~＋CD25~＋Treg in peripheral blood of children with EV71 positive severe hand foot mouth disease（HFMD）.Methods：60 children with severe hand foot mouth disease（HFMD） were selected as the severe group,other 105 cases as the normal group,and 30 healthy children as the control group.The expression of CD4~＋CD25~＋Treg,Th17 and cytokines TGF-β、IL-6、IL-10、IL-17 in each group were monitored.Results：The expression of Th17,IL-6, IL-10 and IL-17 in the severe group was significantly higher than that in the control group（P0.01）,while the expression of CD4~＋CD25~＋Treg and TGF-β was significantly lower than that of the control group（P〈0.01）.Conclusion：The increase of Th17 expression and the decrease of CD4~＋CD25~＋Treg expression in children with hand foot and mouth disease in the severe group,are related to the severity of the disease,which can be used as an important reference index to predict the severity of the disease and the evaluation of the prognosis.
