摘  要:目的观察血浆miR-21和miR-133a在缺血性心肌病(ICM)中的表达水平,同时评价其临床价值。方法采用前瞻性研究方法选择55例ICM患者作为观察组,另选择50例健康体检者作为对照组,比较两组患者的一般资料、相关指标以及血浆miR-21与miR-133a的表达水平,同时观察血浆miR-21与miR-133a表达水平与有关因素的相关性。结果两组的性别构成比、年龄、吸烟史、糖尿病史差异无统计学意义(P>0.05),但观察组PCI术史比率(25.45%)明显高于对照组(0),差异有统计学意义(P<0.05);观察组TG、NT-pro BNP、LVEF、LVEDV水平明显高于对照组,LDL-C、HDL-C、miR-21、miR-133a水平明显低于对照组,差异均有统计学意义(P<0.05),两组TC、收缩压、舒张压差异无统计学意义(P>0.05);观察组的血浆miR-21与NT-pro BNP、LVEDV呈正相关(P<0.05),血浆miR-133a与年龄、TC、TG、LDL-C、HDL-C、NT-pro BNP、LVEF、LVEDV均无相关性(P>0.05)。结论缺血性心肌病患者的血浆miR-21与miR-133a的表达水平较健康人群显著提高;血浆miR-21与NT-pro BNP、LVEDV呈正相关,提示miR-21可能参与了缺血性心肌病患者心室重构过程,可以作为诊断、评估缺血性心肌病的新的生物标志物。Objective To observe expression of plasma miR-21 and miR-133-a in ischemic cardiomyopathy( ICM) and its clinical value. Methods 55 patients with ICM were prospectively selected as observation group,another 50 cases for healthy physical examination as control group. The patients' general information,related indicators,and plasma miR-21 and miR-133-a expression level were compared,at the same time the plasma miR-21 and miR-133-a correlation expression level and relevant factors were observed. Results The gender,age,smoking history,diabetes history showed no significant difference( P〈0. 05),but PCI surgery ratio( 25. 45%) in the observation group was significantly higher than that of the control group with statistically significant difference( P〈0. 05). The level of LVEDV TG,NT-pro BNP and LVEF in the observation group were significantly higher than those of the control group. LDL-C,HDL-C,miR-21 and miR-133 a levels were significantly lower than those of the control group,the differences were statistically significant( P〈0. 05). TC,systolic blood pressure and diastolic blood pressure in the two groups showed no significant difference( P〈0. 05). The plasma miR-21 and NT-pro BNP were positively related with LVEDV in observation group( P〈0. 05),and plasma miR-133 a had no correlation with age,TC,TG,LDL-C,HDL-C,NT-pro BNP,LVEF and LVEDV( P〈0. 05). Conclusion Plasma miR-21 and miR-133-a expression level in patients with ischemic cardiomyopathy are significantly higher than those in healthy people. Plasma miR-21 is positively correlated with the NT-pro BNP,LVEDV,indicating that miR-21 may participate in the ventricular remodeling process in patients with ischemic cardiomyopathy. It can be used as a new biological markers for diagnosis and assessment of ischemic cardiomyopathy.
