摘 要:目的探讨妊娠期糖尿病(GDM)对子代婴儿期体格发育,甲状腺功能,智力发育的影响。方法随机选取2015年1月—2015年12月在苏州市立医院本部产科常规产前检查及足月分娩的妊娠期糖尿病孕妇的子代作为GDM组,同期选取血糖正常的孕妇的子代作为对照组,检测两组婴儿12月龄的体重、身长,促甲状腺激素(TSH)水平,运用Gesell发育量表法检测其智力发育水平。结果 GDM组子代12月龄体重、身长分别为(10.6±1.2)kg、(79.5±4.4)cm,均高于对照组(9.9±1.1)kg、(76.0±2.7)cm,差异有统计学意义。Gesell发育量表评估各能区发育商,GDM组适应性、精细运动、个人—社交发育商均低于对照组,差异有统计学意义;GDM组大运动、语言发育商均低于对照组,但无显著性差异。GDM组促甲状腺激素水平高于对照组,但差异无统计学意义。结论妊娠期糖尿病对子代婴儿期体格发育,智力发育中适应性、精细运动、个人-社交水平存在影响。妊娠期糖尿病对子代婴儿期甲状腺功能无明显影响。Objective To study the effects of gestational diabetes mellitus( GDM) on physical development,thyroid function and intellectual development of offspring during infancy. Methods The offsprings of GDM were randomly selected as the GDM group during routine obstetric examination and full-term delivery in the Affiliated Suzhou Hospital of Nanjing Medical University from January 2015 to December 2015,and the offsprings of pregnant women with normal blood glucose at the same period were selected as the control group. The body weight,body length and thyroid stimulating hormone( TSH) levels were detected at 12 months old in the two groups,and the level of intellectual development was detected by Gesell developmental scale. Results The offspring's weight( 10. 6 ± 1. 2 kg) and length( 79. 5 ± 4. 4 cm) of the GDM group at 12 months old were higher than those( 9. 9 ± 1. 1 kg and 76. 0 ± 2. 7 cm,respectively) of the control group,and the differences were both statistically significant. The Gesell developmental scale was used to evaluate the developmental quotient of each functional area. The adaptability,fine movement and personalsocial developmental quotients in GDM group were lower than the control group, the differences were statistically significant; gross movement,language developmental quotients in GDM group were lower than the control group,but no significant differences were observed between the two groups. The level of TSH in GDM group was higher than the control group,and there was no significant difference. Conclusion GDM has an obvious impact on physical development,adaptability,fine movement and personal-social level of the offspring during infancy. There was no significant effect of GDM on the thyroid function of the offspring.
