基于古代文献回顾的穴位敏化现象和规律研究

摘　　要：目的：文章旨在分析整理古代文献中穴位敏化的现象与规律。方法：从秦汉至近现代77本针灸相关的古医籍中收集穴位敏化的文献记载、医家专论及发挥、临床应用与相关医案,运用文献计量学的研究方法对检索所得文献作定量分析。结果：共检索出与穴位敏化相关的条文101条,通过整理穴位敏化现象,发现穴位敏化以痛敏现象为主。总结穴位敏化现象规律。结果：脏腑病敏化穴位主要以本经经穴、阿是穴以及背俞穴为主,敏化的部位主要分布在本经偱行部位;经络病敏化穴位则是以阿是穴以及病变局部穴位为主,敏化的部位主要分布在局部以及病变经络偱行部位。且根据穴位敏化现象表现的不同可反映疾病的虚实病性、辨别病邪的性质。结论：穴位敏化现象的记载历史悠久,且具有一定的特征与规律,可协助临床诊断,指导临床治疗选穴。Objective：This article aims to analyze and sort out the phenomena and rule of acupoint sensitization in ancient medical literature. Methods：The literature of acupoints sensitization was collected from the 77 involved in acupuncture and moxibustion contents literature from Qin and Han dynasties to the modern,including the literature,the monographs of the doctors,the clinical application and the related medical records. The retrieved literature was quantitatively analyzed by bibliometrics. Results：A total of 101 articles related to acupoints sensitization were retrieved. By arranging the sensitization of acupoints,it was found that the sensitization of acupoints was mainly caused by pain sensitive. After summarizing the rule of acupoint sensitization,the results demonstrated that the sensitized acupoints of Zang-fu diseases were mainly based on its own meridian acupoints,Ashi points and back Shu points. Sensitized parts are mainly distributed on its own meridian. The sensitized acupoints of meridian disease were mainly based on Ashi points and the lesion local acupoints. Sensitized parts are mainly distributed over the local parts of the lesion and its own meridian. And the different manifestations of acupoints sensitization can differentiate the syndromes and property of disease. Conclusion：The record of acupoint sensitization has a long history. And acupoint sensitization phenomenon has certain characteristics and rules and can help clinical diagnosis,guiding the clinical treatment of acupoints selection.
