育阴潜阳汤治疗抗精神病药所致锥体外系不良反应的病例对照研究

摘　　要：目的：探讨育阴潜阳汤对抗精神病药物所致锥体外系不良反应治疗的临床疗效及其不良反应。方法：采用随机双盲病例对照研究设计,将120例精神分裂症患者随机分为治疗组和对照组各60例。治疗组服用育阴潜阳汤2次/d,150 m L/次;盐酸苯海索安慰剂,2次/d,2 mg/次。对照组服用盐酸苯海索2次/d,2 mg/次,育阴潜阳汤安慰剂2次/d,150 m L/次。治疗时间12周,分别于用药前、用药后4周末、8周末和12周末采用锥体外系不良反应量表评估患者的锥体外系不良反应,并在基线和3月末对患者服药依从性进行评估。结果：治疗组对抗精神病药物所致锥体外系不良反应的疗效等同于对照组,组间未发现有显著性差异（F=0.241,P〉0.05）,但不同的观察时间点间差异有显著性（F=80.844,P〈0.05）,不同时间点与组间交互作用不显著（F=0.712,P〉0.05）。服药依从性方面,育阴潜阳汤组优于安坦组,但未达到统计学差异（F=0.992,P〉0.05）,但不同的观察时间点间差异有显著性（F=14.029,P〈0.05）,不同时间点与组别间交互作用显著（F=14.459,P〈0.05）.结论：育阴潜阳汤可以用来治疗抗精神病药物所致的锥体外系不良反应,疗效与盐酸苯海索相当,未见明显的药物不良反应。Objective：To observe the clinical efficacy and side effects of Yuyin Qianyang Decoction in relieving or alleviating the side effects of extrapyramidal drugs induced by antipsychotic drugs. Methods：A randomized double blind case control study was designed. A total of 120 schizophrenic patients were randomly divided into 2 groups. The treatment group（60 cases） was treated with Yuyin Qianyang Decoction,1 dose a day and twic a day. In the control group,60 patients were treated with an oral solution trihexyphenidyl,2 mg each time,twic a day. The 2 groups were observed for 3 months. The extrapyramidal side effects（ RSESE） scale and 2 groups＇ general side effects and medication compliance were assessed before,at the end of 1,2 and 3 months. Results：Yuyin Qianyang Decoction group improved or relieved the extrapyramid effect caused by antipsychotic drugs and its effect was equal to trihexyphenidyl. It showed no significant difference（ F = 0. 241,P〈0. 05） between the groups,but there were significant differences between different time points which were observed at different time points（ F = 80. 844,P〈0. 05）. And group interaction had no correlation（ F = 0. 712,P〈0. 05）. As far as medication compliance,Yuyin Qianyang Decoction group＇s was better than that of control group,without statistical significance（ F = 0. 992,P〈0. 05）,but there was significant difference between different observation time points（ F = 14. 029,P〈0. 05）. The different time point and group interaction was significantly correlated（ F = 14. 459,P〈0. 05）. Conclusion：There are almost no side effects of Yuyin Qianyang Decoction,but the side effects of trihexyphenidyl are more. Yuyin Qianyang Decoction can be used instead of trihexyphenidyl and the curative effect is exact.
