活血潜阳方对H型高血压病人血栓前状态和颈动脉粥样硬化指标的影响

摘　　要：目的观察中药活血潜阳方对H型高血压病人血栓前状态和颈动脉粥样硬化指标的影响。方法选择符合纳入标准的H型高血压病人73例,按随机数字表法分为活血潜阳组(40例)和对照组(33例)。两组病人均给予马来酸依那普利叶酸片1片,每日1次,口服。活血潜阳组加用活血潜阳方,每日1剂,水煎至400mL,分两次,早晚各200mL。所有药物疗程共计8周。观察治疗前后两组病人血压、血同型半胱氨酸(Hcy)、血栓前状态指标[纤维蛋白原(FIB)、血小板α颗粒膜糖蛋白(GMP-140)、D-二聚体(D-Dimer)、抗凝血酶原Ⅲ(AT-Ⅲ)]、颈动脉粥样硬化指标[颈动脉内中膜厚度(IMT)、Crouse积分、颈动脉内径]。结果活血潜阳组降压总有效率高于对照组(P<0.05);两组治疗后Hcy均较治疗前下降(P<0.05),活血潜阳组治疗后Hcy低于对照组(P<0.05);两组AT-Ⅲ治疗后较治疗前均有上升,但差异无统计学意义,而FIB、GMP-140、D-Dimer两组治疗后均较治疗前下降,且治疗后活血潜阳组低于对照组(P<0.05);活血潜阳组治疗后IMT、Crouse积分较治疗前降低差异具有统计学意义(P<0.05),但对颈动脉内径两组治疗前后比较差异无统计学意义(P>0.05)。治疗后活血潜阳组IMT及Crouse积分低于对照组(P<0.05)。结论中药活血潜阳方对血瘀阳亢型H型高血压病人血压、高同型半胱氨酸(HHcy)及其相关血栓前状态及颈动脉硬化均有一定的治疗作用。Objective To observe the clinical effects of Chinese herbs for Huoxue Qianyang formula （HQF） on type H hypertension. Methods Seventy three patients with type H hypertension were randomly divided into two groups：control group （ n =33） treated with enalapril maleate and folic acid tablets,and treatment group （ n =40） treated with HQF plus enalapril maleate and folic acid tablets for 8 weeks.The blood pressure,blood homocysteine （Hcy）,prethrombotic state indicators [fibrinogen （FIB）,platelet granule membrane glycoprotein （GMP 140）,D dimer （D Dimer）,anti prothrombin Ⅲ（AT Ⅲ）],carotid atherosclerosis index [carotid intima media thickness （IMT）,Crouse score,carotid artery diameter] were observed before and after treatment. Results The total effective rate of arterial systolic blood pressure （SBP） and diastolic blood pressure （DBP） decreased in treatment group compared with the control group （ P 〈0.05）.The level of Hcy decreased after treatment （ P 〈0.05）,which was better in treatment group than that in control group （ P 〈0.05）.The levels of FIB,GMP 140,and D Dimer decreased in two groups after treatment,which were lower in treatment group than those in control group （ P 〈0.05）.The IMT and Crouse scores in treatment group after treatment were better than that in control group （ P 〈0.05）. Conclusion HQF has the therapeutic effects on blood pressure,high homocysteine （HHcy） and its related prethrombotic state in patients with type H hypertension and blood stasis and yang hyperactivity syndrome.
