复方苦参注射液联合平溃散保留灌肠治疗急性放射性直肠炎疗效及对机体炎症反应的影响

摘　　要：目的探讨复方苦参注射液联合平溃散保留灌肠治疗急性放射性直肠炎疗效及对机体炎症反应的影响。方法将94例急性放射性直肠炎患者随机分为2组,2组均给予常规对症处理和复方苦参注射液治疗,对照组47例同时给予庆大霉素、地塞米松、生理盐水保留灌肠;观察组47例则给予平溃散保留灌肠,2组每天早晚各灌肠1次,治疗14 d。观察2组治疗前后中医证候积分、肠镜表现、炎症因子水平变化情况,统计2组疗效及不良反应发生情况。结果治疗后,2组各项中医证候评分均显著降低(P均<0.05),且观察组明显低于对照组(P均<0.05);治疗后,2组KPS计分均显著升高(P均<0.05),且观察组明显高于对照组(P<0.05);观察组直肠镜结果中,0度者增多,1度、2度、3度者减少,检查结果显著优于对照组(P<0.05);2组IL-17、IL-23水平均明显降低(P均<0.05),且观察组明显低于对照组(P均<0.05);观察组治疗后总有效率显著高于对照组(P<0.05);2组不良反应发生率比较差异无统计学意义(P>0.05)。结论复方苦参注射液联合平溃散保留灌肠可有效缓解急性放射性直肠炎中医证候,改善直肠黏膜病理状态,减轻炎症反应,且临床应用安全。Objective It is to investigate the effect of Compound Matrine Injection combined with retention enema of Ping Kui San on acute radiation proctitis and its influence on inflammatory reaction.Methods Ninety-four patients with acute radiation proctitis were randomly divided into 2 groups and treated with routine symptomatic treatment and Compound Matrine Injection.On the basic of treatment,forty-seven patients in the control group were given gentamicin,dexamethasone and normal saline for retention enema,and forty-seven cases in the observation group were given Ping Kui San for retention enema.The 2 groups were treated with enema twice per-day,and the treatment course was 14 days.The changes of TCM syndromes,colonoscopy and inflammatory factors in the two groups before and after treatment were observed.The curative effects and adverse reactions of the 2 groups were observed.Results After treatment,the scores of TCM syndromes in the 2 groups were significantly reduced（all P〈0.05）,and the observation group was lower than the control group（P〈0.05）.The scores of KPS in the 2 groups were significantly increased（all P〈0.05）,and the observation group was higher than that of the control group（P〈0.05）.In the observation group,the ratio of 0 degrees was increased,and of 1 degrees,2 degrees and 3 degrees was decreased,and all were significantly better than those in the control group（all P〈0.05）.The levels of IL-17 and IL-23 in the 2 groups were significantly decreased（all P〈0.05）,and the observation group was significantly lower than that of the control group（all P〈0.05）.The total effective rate of the observation group was significantly higher than that of the control group（P〈0.05）.There was no significant difference in the incidence of adverse reactions between the 2 groups（P〉0.05）.Conclusion Compound Matrine Injection combined with retention enema of Ping Kui Powder can effectively relieving improve the TCM syndrome of acute radionuclitis,improve the pathological state of
