摘 要:目的比较锁定加压钢板(LCP)与带锁髓内钉(IIN)治疗肱骨干骨不连的临床疗效。方法手术治疗50例肱骨干骨不连患者,按照使用内固定系统的不同分为LCP组(19例)和IIN组(31例)。比较两组的手术时间、术中失血量、并发症情况、骨折愈合情况及上肢功能的ASAMI评分。结果患者均获得随访,时间12~35个月。术中出血量、愈合时间、愈合率、感染率、骨愈合及关节功能评价两组比较差异均无统计学意义(P>0.05)。但LCP组手术时间及并发症发生情况要少于IIN组(P<0.05)。结论 LCP更符合生物力学的要求,对肩袖及肘关节干扰小,并发症少,是治疗肱骨干骨不连的有效方法。Objective To compare the effect of locking compression plate( LCP) and interlocking intramedullary nail( IIN) in the treatment of humerus shaft nonunion. Methods A total of 50 patients with humerus shaft nonunion had been treated with surgical operation. All patients were divided into LCP group( 19 patients) and IIN group( 31 patients) according to the treatment methods. The operative time,intraoperative blood loss,complications,ASAMI scores of upper limb function and fracture healing were compared between two groups. Results All patients were followed up for 12 ~ 35 months. There were no significant differences between the two groups in terms of intraoperative blood loss,time required for union,union rate,infection rate,bone union and epipodite function score( P 0. 05).However,the LCP group had a faster procedure,less complication incidence rate than the IIN group( P 0. 05).Conclusions LCP is an effective method to treat humerus shaft nonunion,because it is not only more capable of satisfying the requirements of biomechanics,but also able to reduce the disturbance caused to rotator cuffs and elbow joints with a lower possibillity of complications in the treatment process.
