多西他赛联合顺铂和5-氟尿嘧啶诱导化疗方案在局部晚期舌鳞癌中的近期疗效研究

摘　　要：目的探讨术前多西他赛联合顺铂和5-氟尿嘧啶(DPF)诱导化疗方案在局部晚期舌鳞癌中的近期疗效。方法选取160例局部晚期舌鳞癌患者作为研究对象,根据治疗方案不同将患者分为A、B两组。A组(74例)采用DPF方案进行诱导化疗;B组(86例)采用顺铂联合5-氟尿嘧啶(PF)方案进行诱导化疗。将治疗前与诱导化疗结束3周后的原发灶MRI进行比较,观察两种诱导化疗方案的近期疗效。结果(1)A组完全缓解(CR)率、部分缓解(PR)率均高于B组,而无效(NR)率低于B组(P<0.05);(2)A组Ⅰ+Ⅱ级口腔黏膜反应发生率、Ⅲ+Ⅳ级白细胞减少及Ⅰ+Ⅱ级体重减轻发生率高于B组(P<0.05)。两组中性粒细胞缺乏性发热、血红蛋白减少及血小板减少等不良反应发生率比较,差异无统计学意义(P>0.05)。结论采用DPF方案进行局部晚期舌鳞癌术前诱导化疗的近期疗效较好,但不良反应稍高,应积极对症处理。Objective To evaluate the short-term therapeutic effect of Docetaxel combine with Cisplatin and 5-Fluorouracil （DPF） on local advanced tongue squamous carcinoma. Methods Totally 160 patients with tongue squamous cancer were involved in this study. About 74 patients in group A received DPF therapy, while 86 patients in group B was treated with Cisplatin plus 5-Fluorouracil. Clinical manifestations as well as outcome were recorded. Results Patients in group A experienced increased rate of complete release （CR） and partial release and decreased rate of no response （NR） compared with group B （P 〈 0.05）. Side effects including grade III＋Ⅳ leucopenia, grade I ＋ II lose weight and grade III＋Ⅳ oral mucosa reaction was more often in group A when compared with those in group B （P 〈 0.05）, though all side effects were alleviated with specific treatments. No obvious difference in incidence of neutropenia related fever, anemia and thrombocytopenia was identified between two groups. Conclusions DPF for local advanced tongue squamous carcinoma is potentially a promising therapeutic option with increased rate of side effects.
