瘢痕子宫再次妊娠并发前置胎盘对孕妇和围产儿结局的影响

摘　　要：目的:探讨剖宫产后瘢痕子宫再次妊娠并发前置胎盘对孕妇和围产儿结局的影响。方法:以2013年1月至2015年7月广东省东莞市人民医院收治的60例剖宫产后瘢痕子宫再次妊娠并发前置胎盘产妇(观察组)及同期58例无瘢痕子宫妊娠并发前置胎盘产妇(对照组)作为研究对象,均行剖宫产术,观察两组术中出血量、产后2h出血量、产妇及围产儿结局。结果:观察组术中出血量、产后2 h出血量较对照组明显增高(P<0.05);观察组产妇不良结局总发生率为31.67%、围产儿不良结局总发生率为66.67%,较对照组的8.62%、24.14%明显增高(P<0.05)。结论:剖宫产后瘢痕子宫再次妊娠并发前置胎盘产妇存在较高产后大出血风险,危及母婴健康,应对此类患者给予高度重视。Objective： To investigate the impact of cesarean scar pregnancy with placenta previa on the outcome of multipara and perinatal infant. Methods： Included in this study were 60 cases of cesarean scar pregnancy with placenta previa（ study group） treated in Dongguan People＇ s Hospital of Guangdong Province between January 2013 and July 2015,as well as 58 contemporary cases of non-scarred uterine pregnancy with placental previa（ control group）.The both group underwent cesarean section delivery. Intra-operative blood loss,2 h postpartum hemorrhage,maternal and perinatal outcomes were compared between groups. Results： The study group experienced significantly more intraoperative blood loss than the control group（ P〈0. 05）. In the study group,the total incidence rate of adverse outcomes was 31. 67% for the multipara and 66. 67% for the perinatal infant; which were significantly higher than 8. 62% and 24. 14%,respectively,in the control group（ P〈0. 05）.Conclusion： Cesarean scar pregnancy with placenta previa may give rise to a higher risk of major postpartum hemorrhage,endanger the health safety of mothers and babies,and therefore warrants close medical attention.
