参芎葡萄糖注射液联合牵引手法治疗颈性眩晕的临床疗效

摘　　要：目的探讨参芎葡萄糖注射液联合牵引手法治疗颈性眩晕的疗效。方法将310例颈性眩晕患者随机分为治疗组和对照组,每组155例。两组患者均接受14 d的牵引手法治疗,治疗组在此基础上给予参芎葡萄糖注射液静脉滴注。两组治疗前、后使用颈性眩晕症状与功能评估量表(CVSFAS)评估临床疗效并使用TCD超声检查两侧椎-基底动脉的平均血流速度。结果治疗前,两组患者CVSFAS评分比较,差异无统计学意义(P>0.05);治疗后,两组CVSFAS评分均改善,且治疗组优于对照组(P<0.05)。治疗前,两组患者左椎动脉(LVA)、右椎动脉(RVA)、基底动脉(BA)平均血流速度比较,差异无统计学意义(P>0.05);治疗后,两组LVA、RVA、BA平均血流速度均改善,且治疗组明显优于对照组(P<0.05)。结论与普通牵引手法治疗比较,参芎葡萄糖注射液联合牵引手法治疗可明显改善颈性眩晕患者椎-基底动脉血流并缓解眩晕、颈肩痛等临床症状,值得临床推广。Objective To investigate the clinical efficacy of Shenxiong glucose injection combined with traction manipulation in treating patients with cervical vertigo. Methods A total of 310 patients diagnosed with cervical vertigo were randomly divided into treatment group and control group, with 155 cases in each group. Both groups received 14 d traction manipulation therapy. The treatment group was given Shenxiong glucose injection for intravenous drip on this basis. The clinical efficacy of the two groups were evaluated using the Cervical Vertigo Symptoms and Functional Assessment Scale（CVSFAS） before and after treatment, and the average blood flow velocity of both vertebral-basilar arteries were examined using TCD ultrasound. Results There was no significant difference in the CVSFAS scores between the two groups before therapy（P〈0.05）. After treatment, the CVSFAS scores of the two groups improved, and those in the treatment group were significantly higher than the control group（P〈0.05）. There were no significant differences in mean blood flow velocity of left vertebral artery（LVA）, right vertebral artery（RVA） and basilar artery（BA） between the two groups before therapy（P〈0.05）. After treatment, the average blood flow velocity of LVA, RVA and BA of the two gruops improved, and those in the treatment group were higher than the control group（P〈0.05）. Conclusion Compared with ordinary traction manipulation therapy, the Shenxiong glucose injection combined with traction manipulation can significantly improve the vertebrobasilar artery blood flow in patients with cervical vertigo and relieve clinical symptoms such as dizziness, neck and shoulder pain,and is worthy of clinical promotion.
