超声在胆囊隆起性病变中的诊断价值

摘　　要：目的探讨超声在胆囊隆起性病变(PLG)中的诊断价值。方法回顾性分析896例PLG患者的临床资料,根据病理结果将胆囊息肉患者作为A组,胆囊腺肌症患者作为B组,胆囊腺瘤患者作为C组,胆囊癌患者作为D组。分析各组患者的临床表现及B超检查影像学表现。结果经病理诊断证实,A组834例,B组35例;C组17例;D组10例。超声诊断PLG与病理检查符合率为88.84%(796/896)。A组患者PLG血流情况与B、C、D组比较,差异具有统计学意义(P<0.05)。D组患者病变直径明显大于其他三组(P<0.05)。结论超声诊断是PLG的常用影像诊断技术之一,可作为胆囊癌前病变的预防依据。Objective To evaluate the diagnostic value of ultrasound in the diagnosis of polgpoid lesions of the gallbladder（PLG）. Methods The clinical data of 896 patients with PLG were retrospectively analyzed. According to the pathological results, the patients with gallbladder polyps were selected as group A, patients with gallbladder adenomyosis were enrolled as group B, patients with gallbladder adenoma were collected as group C, and patients with gallbladder carcinoma were selected as group D. The clinical manifestations and B-ultrasound imaging findings of each group were analyzed. Results According to pathological diagnosis, there were 834 cases in the group A, 35 cases in the group B, 17 cases in the group C, and 10 cases in the group D. The coincidence rate of PLG between ultrasound diagnosis and pathological examination was 88.84%（796/896）. The blood flow of PLG in the group A was significantly different from that in the group B, group C and group D（P〈0.05）. The diameter of lesions in the group D was significantly greater than that in the other three groups（P〈0.05）. Conclusion Ultrasound is one of the common diagnostic techniques of PLG, which can be used as the basis for the prevention of the PLG.
