摘　　要：目的探讨髂腹股沟真皮下血管网皮瓣在手背皮肤缺损的临床疗效。方法自2015年8月-2018年5月我院急诊收治17例手背皮肤缺损患者,其中合并肌腱断裂6例,掌指骨骨折10例,均给予一期清创骨折固定和肌腱修复术,3例创面相对清洁一期腹股沟真皮下血管网皮瓣修复,其余14例均VSD负压吸引,5-14天后行腹股沟真皮下血管网皮瓣修复术。结果皮瓣全部成活,术后随访6-20个月,皮瓣质地良好,外形满意,手部功能良好。结论腹股沟真皮下血管网皮瓣修复手背皮肤缺损,具有皮瓣外形美观,切取简单,供区隐蔽可直接缝合,血供可靠及抗感染能力强等优点。Objective To investigate the clinical effect of ilioinguinal subdermal vascular network flap for skin defect of the back of the hand. Methods 17 cases of skin defect on the back of the hand were admitted by our emergency department since August 2015 to May 2018. Among them, 6 cases were complicated with concomitant tendon rupture and 10 cases accompanied with metacarpus and phalange fracture, all of them were treated with one stage debridement fracture fixation and tendon repair, 3 cases had a relatively clean wound surface were repaired primary ilioinguinal subdermal vascular network flap, the remaining 14 cases were attracted by VSD negative pressure ilioinguinal subdermal vascular network flap repair 5-14 days later. Results All the flaps survived, and the postoperative follow-up was 6-20 months. The flaps had good texture, satisfactory appearance and good hand function. Conclusion The inguinal subdermal vascular network flap repairs the skin defect of the back of the back of the hand. It has the advantages of beautiful appearance of the flap, simple cutting, direct suturing of the donor area, reliable blood supply and strong antiinfective ability.
