260例慢性肺源性心脏病的心电图观察

摘　　要：目的探讨慢性肺源性心脏病患者的心电图诊断要点。方法选择2014年1月~2016年12月我院收治的260例慢性肺源性心脏病患者,对所有患者行12导联心电图分析,并对患者的所有其他临床资料进行回顾性分析。结果肺心病患者检出肺型P波为174例(66.9%),ptfv1异常81例(31.0%),电轴右偏130例(50.0%),低电压56例(24.3%),顺钟向转位138例(53.1%),心律失常156例(60.0%),以房性心律失常多见,ST-T改变共80例(30.8%)。结论慢性肺源性心脏病患者的心电图表现具有一定特征性,依据患者的症状、体征、实验室检查,结合患者心电图的特征性改变,对明确肺心病的诊断及指导其治疗具有一定价值。Objective To analyze the key points of electrocardiogram diagnosis of patients with chronic pulmonary heart disease. Methods 260 cases of chronic pulmonary heart disease admitted to our hospital from January 2014 to December 2016 were selected. All patients were analyzed by 12 lead electrocardiogram, and all other clinical data of patients were retrospectively analyzed. Results The pulmonary P wave was detected in 174 cases（66.9%）, abnormal PTFV1 in81 cases（31.0%）, 130 cases（50.0%） in the right axis, 56 cases（24.3%） in low voltage, and 138 cases in the clockwise transposition（53.1%）. Arrhythmia in 156 cases（60.0%）, common in atrial arrhythmia, ST-T changes 80 cases（30.8%）.Conclusion The electrocardiogram of patients with chronic pulmonary heart disease has certain characteristics. According to the patients′symptoms, signs and laboratory tests, combined with the characteristic changes of electrocardiogram,it is valuable for the diagnosis and treatment of cor pulmonale.
