血必净联合乌司他丁治疗老年重症感染53例临床观察  

Clinical Observation on Xuebijing Injection Combined with Ulinastatin in the Treatment of 53 Elderly Patients with Severe Infection

摘　　要：目的观察血必净注射液联合乌司他丁治疗老年重症感染的临床疗效。方法采用分层随机化法,将2015年3月至2017年5月医院重症监护室(ICU)的老年重症感染患者105例分为观察组(53例)和对照组(52例),两组患者给予血必净注射液治疗,观察组患者加用乌司他丁,均连续用药14 d。结果治疗后,两组患者白细胞计数(WBC)、降钙素原(PCT)、C反应蛋白(CRP)水平均明显下降,且观察组下降更明显(P<0.05);两组白细胞介素6(IL-6)、肿瘤坏死因子-α(TNF-α)较治疗前降低,白细胞介素10(IL-10)升高,且观察组变化更显著(P<0.05);观察组平均住院时间、急性生理学及慢性健康状况评分(APACHEⅡ)评分均较对照组显著减少(P<0.05);观察组总有效率为94.34%,明显高于对照组的80.77%,观察组死亡率为3.92%,明显低于对照组的15.38%,差异均有统计学意义(P<0.05)。结论血必净注射液联合乌司他丁治疗老年重症感染临床疗效显著,可有效降低炎性因子水平及死亡率,临床价值显著。Objective To observe the clinical effect of Xuebijing Injection combined with ulinastatin in the treatment of elderly patients with severe infection. Methods Totally 105 elderly patients with severe infections in ICU from March 2015 to May 2017 were divided into the observation group（ n =53） and the control group（ n =52） by the stratified randomization method. The patients in the two groups were treated with Xuebijing Injection,on this basis,the patients in the observation group were treated with Ulinastatin,they were treated with 14 d continuously. Results After treatment,the levels of WBC,PCT and CRP in the two groups were significantly decreased,and the decrease in the observation group was more significant（P 〈 0. 05）. The levels of IL-6 and TNF- α in the two groups were significantly decreased than those before treatment,while the levels of IL-10 in the two groups were significantly increased than those before treatment, and the change in the observation group was more significant（ P 〈 0.05）. The average hospitalization time and APACHE Ⅱ score in the observation group were significantly decreased than those in the control group（P 〈 0.05）.The total effective rate of the observation group was 94. 34% ,which was significantly higher than 80.77% of the control group（P 〈 0.05）. The mortality rate of the observation group was 3.92% ,which was significantly lower than 15.38% of the control group（P 〈 0. 05）. Conclusion Xuebijing Injection combined with ulinastatin in the treatment of elderly patients with severe infections has significant clinical effect,it can effectively reduce the levels of inflammatory factors,reduce mortality rate,which has significant clinical value.
