摘　　要：目的确保患者静脉输液安全有效。方法临床药师审核2012年9月至2013年12月医师计算机提交的675 288组输液处方,统计不合理医嘱。结果发现不合理医嘱324组,包括溶剂品种错误240组,超剂量用药32组,溶剂超量14组,未皮试13组,药物浓度较高11组,滴速过快10组,配伍禁忌2组等;经药师干预,不合理处方全部得以纠正。结论药师审核输液处方可纠正不合理处方,保障患者静脉输液安全合理,并可杜绝或减少医院不合理输液导致的用药纠纷。Objective To ensure the safety and effectiveness of intravenous infusion. Methods Clinical pharmacists reviewed 675 288 groups of infusion prescriptions submitted by physicians＇ computers from September 2012 to December 2013,and counted irrational doctor＇s orders. Results Totally 324 groups of irrational doctor＇s orders were found, including 240 groups of solvent varieties error, 32 groups of over dosage,14 groups of over solvent,13 groups of non-skin test,ll groups of higher drug concentration,10 groups of rapid intravenous drip,and 2 groups of incompatibility. All irrational prescriptions were corrected by pharmacist intervention. Conclusion The pharmacists can correct the irrational prescriptions by reviewing the infusion prescription,ensure the safety and rationality of intravenous infusion,and prevent or reduee the drug disputes caused by unreasonable infusion in the hospital.
