慢性盆腔痛患者依恋心理与疼痛感的关系研究

摘　　要：目的调查慢性盆腔痛患者的依恋心理现状,并分析其在整体疼痛感中的作用。方法选取2017年2月至2017年8月在浙江省温州市两所三甲医院就诊的慢性盆腔痛患者作为研究对象。使用亲密关系体验问卷和整体疼痛评估量表收集资料。结果研究对象依恋回避得分为3.16±0.88分,依恋焦虑得分为3.53±0.96分。安全型依恋模型人数为134人(39.0%)。整体疼痛得分为77.41±13.42分。经分层逐步回归分析,是否独居、发作频率和依恋焦虑可影响患者整体疼痛感,且可解释总变异的32.6%。结论医护人员应关注慢性盆腔痛患者的心理状况,对于亲密关系需求强烈者,应做好家属的教育工作,给予足够的关注和理解,以促进患者对疼痛的积极调适,提高生活质量。Objective To investigate the attachment in patients with chronic pelvic pain, and to analyze its effect on global pain feeling. Method The patients with chronic pelvic pain who were treated at the 2 class-A tertiary hospitals in Wenzhou, Zhejiang Province from February 2017 to August 2017 were selected as the study subjects. Experiences in Close Relationships Questionnaire-Revised and Global Pain Scale were used to collect data. Results The scores of attachment avoidance and attachment anxiety were 3.16±0.88 and 3.53±0.96, respectively. There were 134（42.5%） participants of secure attachment. The score of global pain was 77.41±13.42. After hierarchical stepwise regression analysis, living alone or not, pain frequency and attachment anxiety were the influencing factors of global pain, explaining 32.6% of the total variation. Conclusion Medical staff should pay attention to the psychological status of patients with chronic pelvic pain. For those who have a strong need for intimacy, medical staff should carried out family education and give enough attention and understanding to promote the patients＇ active adjustment of pain and improvement of quality of life.
