摘　　要：目的:探讨肝硬化患者行腹腔镜胆囊切除术(laparoscopic cholecystectomy,LC)中保留胆囊床部分胆囊后壁的安全性。方法:回顾分析2014年7月至2017年7月为80例胆囊结石患者行LC的临床资料,其中肝硬化患者40例(观察组),术中保留胆囊床部分胆囊后壁;无肝硬化患者40例(对照组),行常规LC。对比分析两组手术时间、术中出血量、术后腹腔引流时间、术后住院时间、住院费用及术后并发症。结果:两组手术时间、术中出血量、术后腹腔引流时间、术后住院时间、住院费用及术后并发症发生率差异无统计学意义(P>0.05)。结论:肝硬化Child A级患者行LC术中保留胆囊床部分胆囊后壁是安全的,患者创伤小,术后康复快,并发症少。Objective ： To investigate the safety of laparoscopic cholecystectomy with the preservation of the gallbladder wall of the gallbladder bed in the patients with liver cirrhosis. Methods： The clinical data of 80 patients with cholecystolithiasis who underwent laparoscopic cholecystectomy from Jul. 2014 to Jul. 2017 were retrospectively analyzed. 40 patients with liver cirrhosis who underwent laparoscopic cholecystectomy with the preservation of the gallbladder wall of the gallbladder bed were in observation group, and 40 pa-tients without liver cirrhosis were in the control group, then the operation time,intraoperative bleeding, postoperative abdominal drainage time, postoperative hospital stay, hospitalization expenses and postoperative complications were compared between the two groups. Re-sults： There was no significant difference in operative time, intraoperative blood loss, postoperative abdominal drainage time, postopera-tive hospital stay, hospitalization expenses or postoperative complication incidence between the two groups （ P 〉 0.05 ）. Conclusions ： The application of laparoseopic cholecystectomy with the preservation of the gallbladder wall of the gallbladder bed is safe and mini-in- vasive with quick recovery and few postoperative complications for the patients with liver cirrhosis and Child-Pugh A liver function.
