摘 要:目的研究分析彩色多普勒超声对中孕期胎儿心脏畸形的筛查价值。方法回顾2015年6月至2017年12月期间,我院共有60例孕妇行中孕期胎儿心脏畸形筛查,对所有胎儿均采用彩色多普勒超声筛查。观察四腔心与左(右)室流出道及三血管气管切面,主动脉弓、导管弓及上腹部横切面的筛查结果。结果彩色多普勒超声诊断的灵敏度92.68%,特异度89.47%,漏诊率5.00%,阳性预测值95%,阴性预测值85%;产前筛查与产后随访检出率、畸形类型比较无明显差异性(P>0.05)。结论彩色多普勒超声能有效筛查中孕期胎儿心脏先天性畸形,综合筛查准确率较高,值得临床借鉴。Objective:To analyze the value of color Doppler ultrasonography in screening fetal heart malformations during the second trimester. Methods:From June 2015 to December 2017,a total of 60 pregnant women were screened for fetal cardiac malformation during the middle pregnancy,and all the fetuses were screened by color Doppler ultrasound. The screening results of four chamber heart and left(right)ventricular outflow tract and three vessel trachea section,aortic arch,duct arch and transverse section of upper abdomen were observed. Results:The sensitivity of color Doppler ultrasound diagnosis was 92.68%,specificity 89.47%,missed diagnosis rate 5%,positive predictive value 95%,negative predictive value 85%. There was no significant difference in the detection rate between antenatal screening and postpartum follow-up(P〉0.05). Conclusion:Color Doppler ultrasound can effectively screen the congenital malformation of fetal heart during pregnancy,and the accuracy of comprehensive screening is high. It is worthy of clinical reference.
