室性心动过速积分与宽QRS心动过速鉴别诊断

摘　　要：室性心动过速(室速)积分是宽QRS波群心动过速鉴别诊断的一种新方法,该方法综合7项心电图指标(V_1导联初始R波、V_1或V_2导联起始r波>40ms、V_1导联S波有挫折、aVR导联起始R波、Ⅱ导联RWPT≥50ms、V_1～V_6无RS型、房室分离),采用评分方法对宽QRS波群心动过速进行鉴别,鉴别诊断截断值≥3分,即室速积分≥3分者为室速,积分=2分者为可疑室速,积分=0分者考虑为室上性心动过速。积分越高,室速诊断的可靠性越大。The ventricular tachycardia score is a new method for the differential diagnosis of wide QRS complex tachycardia.This method integrated seven items of electrocardiogram,including the initial R wave of lead V_1,the r wave40 ms in V_1 or V_2 lead,frustration of the S-wave in V_1 lead,the initial R wave in aVR lead,RWPT ≥50 ms in the II lead,without RS-type in V_1-V_6 lead,and atrioventricular separation.The wide QRS complex tachycardias were identified and diagnosed by using the ventricular tachycardia score method.The cut-off value of the ventricular tachycardia score is≥3 points.Patients with ventricular tachycardia score≥3 points can be diagnosed with ventricular tachycardia,while patients may be suspected with ventricular tachycardia when the ventricular tachycardia score was 2 points.Besides,patients may develop SVT when the score is 0 point.The higher ventricular tachycardia score was positively correlated with the reliability of VT diagnosis.
