高血压患者脉压变化与心电图异常的相关性

摘　　要：目的探讨高血压患者脉压(pulse pressure,PP)变化与心电图异常的关系。方法选取我院2015年6月～2017年6月门诊及住院治疗的300例高血压患者列入研究,对患者进行血压测量以及心电图检查,并按照脉压≤60mmHg、61mmHg≤脉压≤80mmHg、81mmHg≤脉压≤100mmHg以及脉压≥100mmHg将所有患者分成四组,探讨分析脉压与患者的年龄、空腹血糖(fasting plasma glucose,FPG)、甘油三酯(triglyceride,TG)、体重指数(body mass index,BMI)、心率(heart rate,HR)、血清总胆固醇(total cholesterol,TC)、舒张压(diastolic blood pressure,DBP)、收缩压(systolic blood pressure,SBP)、平均动脉压(mean arterial pressure,MAP)以及心电图异常发生率之间的关联性。结果伴随着患者PP的增加,其年龄、SBP、MAP、心电图异常发生率数值均逐渐增加,且四组间比较p<0.05;而DBP数值逐渐降低,且p<0.05;另外,患者PP的增加,其FPG、BMI、HR、TG、TC等数值并无明显变化,p>0.05。结论高血压患者脉压变化与心电图异常之间具有着密切关联性,在临床中,可将高血压患者脉压数据作为其发生心电图异常的主要参考依据。Objective To investigate the relationship between changes of pulse pressure（PP） and abnormal ECG in hypertensive patients. Methods 300 hypertensive patients in our hospital from June 2015 to June 2017 were included in the study. Blood pressure measurement and electrocardiogram（ECG） examination were performed.According to the pulse pressure ≤ 60 mmHg, 61 mmHg ≤ pulse pressure ≤80 mmHg, 81 mmHg≤pulse pressure≤100 mmHg and pulse pressure ≥100 mmHg. All the patients were divided into four groups to investigate the relationship between pulse pressure and the patient＇s age, fasting plasma glucose（FPG）, triglyceride（TG）, body mass index（BMI）, heart rate（HR）, total cholesterol（TC）, diastolic blood pressure（DBP）, systolic blood pressure（SBP）,mean arterial pressure（MAP） and incidence of electrocardiogram abnormalities. Results With the increase of PP, the age, SBP, MAP and ECG abnormalities increased gradually with the difference of p〈0.05 between the four groups;DBP decreased gradually and p〈0.05; in addition, the increase of PP, The FPG, BMI, HR, TG, TC and other values did not change significantly, p〉0.05. Conclusion There is a close relationship between pulse pressure changes and abnormal ECG in patients with hypertension. In clinical practice, pulse pressure data of patients with essential hypertension can be used as the main reference for ECG abnormalities.
