左室收缩功能正常的心房颤动人群血浆BNP水平的影响因素

摘　　要：目的探讨左室收缩功能正常的心房颤动患者血浆B型脑钠肽(BNP)水平的变化及影响因素。方法收集本院2014年1月至2015年12月心血管内科及老年病房收治的180例左室射血分数(LVEF)≥50%患者资料,根据是否合并心房颤动分为心房颤动组和对照组,每组90例。比较两组血浆BNP水平及超声心动图检查指标。结果心房颤动组BNP自然对数(LnBNP)为4.32±1.09,左房前后径(LAD)为(40.8±6.0)mm,明显高于对照组的2.84±0.81、(36.4±4.5)mm,差异有统计学意义(均P<0.001)。Logistics回归分析显示,患者BNP水平与心房颤动的发生密切相关(P<0.001)。多元回归分析发现,患者年龄、LAD、体质量指数(BMI)及甘油三酯(TG)与LnBNP相关(P<0.001)。结论在LVEF≥50%的人群中,心房颤动可引起血浆BNP水平升高,随年龄增长其升高趋势更为明显,LAD大小对BNP水平有一定的预测价值。Objective To study the influence factors of plasma brain natriuretic peptide（BNP） level in atricular fibrillation patients with normal left ventricular systolic function.Methods 180 patients with left ventricular ejection fraction（LVEF）≥50% were collected,and divided into two groups by whether or not complicated with atricular fibrillation（AF）,AF group with 90 patients and non AF group with 90 patients,and plasma BNP level and echocardiography parameters were compared between the two group.Results ln BNP and left atrial diameter（LAD） were significantly higher in AF group than in non AF group（4.32±1.09 vs.2.84±0.81,40.8±6.0 mm vs.36.4±4.5 mm,all P〈0.001）.Logistic regression analysis showed patients plasma BNP level were significantly correlated with incidence of AF（OR：1.045,P〈0.001）.Further multiple regression analysis the influence factors of plasma BNP level in AF group,showed age,LAD,body mass index（BMI） and triglyceride（TG） were correlated with ln BNP（total correlation coefficient r=0.576,P〈0.001）,though TG were entering the model,but have not statistically significant（P=0.074）.Conclusion In patients with LVEF≥50%,plasma BNP level are elevated in AF group,and will increase with aging.LAD has important value in predicting the plasma BNP level.
