摘 要:目的观察回授法对老年糖尿病患者胰岛素笔注射技巧及行为的影响。方法便利抽样法选取2017年2—6月和2017年7—12月,在本院内分泌科住院60岁以上初次胰岛素治疗的2型糖尿病患者为研究对象。2017年2—6月收集的72例患者为对照组,2017年7—12月收集的63例为实验组。对照组按照常规方法教育,由责任护士按照胰岛素笔注射操作教育流程进行教学示范。实验组采用回授法健康教育。入组时和出院前一天,对患者的胰岛素笔注射技巧、胰岛素注射相关知识调查问卷评分,遵医行为进行比较。分析回授法对老年糖尿病患者胰岛素笔注射技巧及行为的影响。结果实验前,两组患者胰岛素笔注射技巧、胰岛素知识调查问卷评分的差异均无统计学意义(P>0.05),实验后两组患者胰岛素笔注射技巧、胰岛素知识调查问卷评分均有所提高,差异均有统计学意义(t=48.11,25.77,P<0.001),但实验组评分明显高于对照组(t=9.92,P<0.001),遵医行为实验组与对照组比较有显著性差异(χ~2=21.7,P<0.001)。说明采用回授法对老年糖尿病患者进行胰岛素注射技巧教育,能明显提高患者对注射技巧及知识的掌握程度,提高遵医行为。结论回授法能够使老年糖尿病患者熟练掌握胰岛素笔注射技巧及胰岛素注射相关知识,提高遵医行为,值得在临床教育中推广应用。Objective To investigate the influence of the feedback method on the injection technique of novopen and behavior of elderly patients with diabetes mellitus.Methods Convenient sampling method was used to select 60 cases of elderly patients ( elder than 60 years old) those who were newly diagnosed type 2 diabetes as the study subjects. The patients diagnosed during the February and June 2017 were selected as the control group (n= 60), and the patients diagnosed during July to December 2017 were selected as experimental group (n = 63). Patients in the control group were educated by the conventional method, that the duty nurses give the demonstration lesson according to the operating educational process of novopen. The experimental group adopted the Feedback Method. At the time of entering the group and on the day before discharge, the patient's insulin pen injection technique, the scores of relevant knowledge questionnaire of insulin injection, and the compliance behavior of the two groups were compared. Analyze the effect of feedback method on insulin injection technique and behavior in elderly diabetic patients. Results Before the experiment, there was no significant difference between the two groups in the insulin pen injection techniques and score of insulin knowledge questionnaire ( P〉0. 05 ). After the experiment, the techniques of insulin injection and scores of insulin knowledge questionnaire of the two groups increased significantly ( t = 48.11, 25.77, P〈 0.001 ) , and the scores of the experimental group were significantly higher than the control group (t = 9.92, P〈0.001 ). The difference of compliance behavior between the two groups was statistically significant (χ2= 21.7,P〈0.001 ). It is suggested that using the feedback method to educate the elderly diabetic patients about insulin injection technique can obviously improve the patient's mastery of injection technique and knowledge, and improve the behavior of compliance.
