摘 要:目的描述北京市首例由长湾泥沙门菌引起的感染性腹泻病例的发病就诊过程和流行病学特征,为有效防控此类病例的发生提供依据。方法通过中国疾病预防控制信息系统及时发现病例,对该病例进行流行病学调查,采集病例的血液和粪便标本进行实验室检测,根据流行病学调查资料分析可能存在的危险因素。结果患儿女性,6月龄,2017年6月15日因发热、呕吐和腹泻等症状入院治疗,经北京市疾病预防控制中心检测患儿为长湾泥沙门菌阳性(Salmonella Jangwani,17:a:1,5)。流行病学调查显示,巴西红耳龟可能是造成本次感染的传染源,经污染环境和手的接触传播可能是造成本次感染的传播途径,患儿日常护理卫生情况可能是危险因素。结论该病例是北京市首例由长湾泥沙门菌引起的感染性腹泻病例;同时这一菌株的发现对增加北京市沙门菌血清型的多样性和生物溯源研究具有重要的公共卫生学意义;对此病例开展的流行病学调查和实验室检测,为今后对此类腹泻疫情的规范处置提供了依据。[Objective] To describe the pathogenic process and epidemiological characteristics of the first case of infectious diarrhea caused by Salmonella Jangwani in Beijing,and provide the evidence for effective prevention and control of such cases in the future. [Methods] The case was reported through the China information system for disease control and prevention in time.Epidemiological investigation of the case was conducted and the blood and stool samples were collected for laboratory testing. The risk factors were analyzed according to epidemiological survey data. [Results] The patient child was female,6 months old,suffered a sudden onset of fever with emesis and diarrhea on June 15,2017,and admitted to hospital. Beijing CDC detected in children with Salmonella Jangwani positive(17:a:1,5). The epidemiological survey showed that the Brazilian turtle might be a source of Salmonella Jangwani infection. The polluted environment and hands might be the route of transmission of this infection.The daily hygiene condition might be the risk factor. [Conclusion] This patient is the first infectious diarrhea case induced by Salmonella Jangwani infection in Beijing. The discovery of this strain has important public health significance in increasing the serotype diversity and biological traceability of salmonella in Beijing. The epidemiological survey and laboratory detection for this case provides the scientific basis for the future normative disposal of such diarrhea epidemic situation.
