摘 要:目的系统评价母亲声音干预(Maternal sound intervention,MSI)对早产儿临床结局的影响。方法检索CENTRAL(Cochrane Central Register of Controlled Trials),PubMed,CINAHL,MEDLINE,Web of Science,中国知网(CNKI),万方数据库,收集采用MSI应用于早产儿的随机对照试验及随机交叉试验,数据库检索时限均为建库至2017年11月。由2名评价者按照纳入与排除标准独立筛选文献、提取资料和质量评价,交叉核对后,采用Review Manager 5.3软件进行Meta分析。结果最终纳入9项研究,包括7篇随机对照试验,2篇随机交叉试验,共397例研究对象。Meta分析结果显示:MSI组早产儿干预时心率[MD=-7.01,95%CI(-9.06^-4.96),P<0.001]及干预后心率[MD=-8.93,95%CI(-11.71^-6.14),P<0.001]低,干预时血氧饱和度[MD=1.27,95%CI(0.23~2.31),P<0.001]及干预后血氧饱和度[MD=1.91,95%CI(0.57~3.25),P<0.001]更高;在呼吸频率,喂养效率,过渡时间,住院时间,每日体质量增长方面MSI没有明显效果(P>0.05),尚不能形成定论。结论MSI能有效降低早产儿心率,增加其氧饱和度,可应用于临床。Objective to evaluate the effects of maternal sound intervention(MSI)on clinical outcomes in preterm infants systematically.Methods We searched from inception to November 2017 in CENTRAL(Cochrane central register of controlled trials),PubMed,CINAHL,MEDLINE,Web of Science,China National Knowledge Internet(CNKI)and Wanfang databases,and collected the randomized controlled trials and randomized crossover trials about the MSI for premature infants.Depending to the inclusion and exclusion criteria,2 reviewers screened the literature,extracted data and evaluated the quality independently.After cross check,a meta analysis was undertaken by the review manager 5.3 software.Results 9 studies were identified,including 7 randomized controlled trials and 2 randomized crossover trials with a total of 397 participants.The results of Meta analysis showed that MSI group got lower heart rate during intervention[MD=-7.01,95%CI(-9.06~-4.96),P〈0.001]and after intervention[MD=-8.93,95%CI(-11.71~-6.14),P〈0.001],and lower oxygen saturation during intervention[MD=1.27,95%CI(0.23~2.31),P〈0.001]and after intervention[MD=1.91,95%CI(0.57~3.25),P〈0.001];No significant difference was identified in terms of feeding efficiency,respiratory rate,transition time,hospitalization time and daily weight gain between two groups(P〈0.05),and the effects were still not formed.Conclusion MSI can reduce the heart rate and increase oxygen saturation in premature infants effectively.Therefore it is suitable for clinical application for premature infants.
