摘　　要：目的探讨可调式支撑喉镜下声门暴露困难患者喉显微外科手术的应用。方法回顾性分析2014年12月至2017年11月经术前评估及术中暴露困难确诊为声门暴露困难的42例患者经可调式支撑喉镜下行喉显微外科手术,其中,任克氏水肿4例者,声带囊肿2例,声带息肉36例。结果 42例患者中,39例在可调式支撑喉镜下顺利完成手术;但3例患者因暴露局限,其中2例喉显微器械辅以角度鼻内镜完成息肉摘除,1例改行表麻电子喉镜下手术。结论可调式支撑喉镜或结合鼻内镜是治疗声门暴露困难患者有效的喉显微外科术式。Objective To explore the application of microsurgery under adjustable support laryngoscope in difficult laryngeal exposure case. Methods A retrospective analysis was performed on 42 patients who were diagnosed with difficult laryngeal exposure during the preoperative or intraoperative evaluation from December 2014 to November 2017. There are 4 cases of reinke＇s edema, 2 cases of vocal cyst and 36 cases of vocal polyp. Results In 42 patients, 39 cases were successfully completed surgery under the adjustable support laryngoscope. However, 2 cases were operated under nasal endoscopy due to exposure limitation and one case with vocal cord polyp was operated under electronic laryngoscope with topical anesthesia. Conclusion The adjustable support laryngoscope or combined with nasal endoscopy is an effective microsurgery procedure for patients with difficult laryngeal exposure.
