摘 要:目的评估应用超声二维斑点追踪(2D-STI)技术评价左室壁收缩期应变值的变化,并评估2D-STI技术在诊断冠心病(CHD)中的临床应用价值。方法采用2D-STI技术检测和分析60例健康人(对照组)和80例CHD患者(CHD组)左室17节段收缩期二维纵向应变值(LS),并与冠状动脉造影进行对比分析,评价2D-STI技术与冠状动脉狭窄的相关性,并绘制受试者工作特征(ROC)曲线检测LS检出冠心病的灵敏度和特异度。结果 CHD患者左室壁收缩期径向LS值明显低于对照组(P<0.05)。以LS为-17.90为截断值诊断CHD的灵敏度为86.31%,特异度为72.59%。2D-STI技术诊断CHD的阳性检出率高于2DE(P<0.05)。结论随着CHD患者冠脉狭窄程度的增加,其左室纵向LS值均减弱,2D-STI技术可以快速、无创地评价左室壁17节段LS,可以作为CHD患者筛查的一种重要方法。Objective To evaluate the changes of left ventricular systolic strain in patients with coronary heart disease(CHD) by two-dimensional speckle tracing (2D-STI), and to evaluate the clinical application value of 2D-STI in thediagnosis of CHD. Methods 2D -STI was used to detect and analyze the two -dimensional longitudinal strain of leftventricular systolic phase in 60 healthy subjects (control group) and 80 patients with coronary heart disease (CHD group),and to evaluate the correlation between 2D-STI technique and coronary artery stenosis. The sensitivity and specificity of LSin the detection of CHD were detected by drawing the receiver operating characteristic (ROC) curve. Results The radialstrain of left ventricular wall in CHD group was lower than that in the control group (P〈0.05). When LS was -17.90 as thetruncation value for screening stenosis, the diagnostic sensitivity was 86.31% and the specificity was 72.59%. The positivedetection rate of 2D-STI in the diagnosis of CHD was higher than 2DE (P〈0.05). Conclusion With the increase of coronarystenosis in CHD patients, the longitudinal strain of left ventricle is weakened. 2D-STI can quickly and non -invasiveevaluate LS in 17 sections of the left ventricle wall, which can be used as an important method for screening CHD patients.
