改良鼻丘径路手术联合鼻甲切除术在难治性鼻窦炎患者中的应用

摘　　要：目的研究改良鼻丘径路手术联合鼻甲切除术在难治性鼻窦炎患者中的应用效果。方法将92例难治性鼻窦炎患者随机分为对照组和联合组,各46例。对照组予以改良鼻丘径路手术治疗,联合组予改良鼻丘径路手术联合鼻甲切除术。比较两组术前、术后1个月的症状评分,对比两组的治愈率、并发症发生情况和复发率。结果术后1个月,两组的鼻塞、脓涕、头痛症状评分明显降低,且联合组均低于对照组(P<0.05);联合组的治愈率高于对照组,复发率低于对照组(P<0.05),两组的并发症总发生率差异不显著(P>0.05)。结论改良鼻丘径路手术联合鼻甲切除术能有效改善难治性鼻窦炎患者的症状,提高治愈率,降低复发率。Objective To study the application effect of modified nasal hill surgery combined with turbinectomy inpatients with refractory sinusitis. Methods A total of 92 patients with refractory sinusitis were randomly divided into controlgroup and combined group, with 46 cases in each group. The control group was treated with modified nasal hill surgery,while the combined group was treated with modified nasal hill surgery combined with turbinectomy. The symptom scores ofthe two groups before surgery and1 month after surgery were compared; the cure rates, the incidence of complications andthe recurrence rates of the two groups were compared. Results After 1 month surgery, the symptom scores of nasalobstruction, pus snot and headache in the two groups significantly decreased, and those in the combined group were lowerthan the control group （P〈0.05）. The cure rate of the combined group was higher than that of the control group, therecurrence rate was lower than that of the control group（P〈0.05）. There was no significant difference in the total incidencerate of complications between the two groups（P〉0.05）. Conclusion The application of modified nasal hill surgery combinedwith turbinectomy in the treatment of refractory sinusitis can effectively improve the symptoms of the patients, improve thecure rate and reduce the recurrence rate.
