托拉塞米联合甲基泼尼松龙治疗肾病综合征并发急性肾衰竭的临床疗效

摘　　要：目的探究托拉塞米联合甲基泼尼松龙治疗肾病综合征(NS)并发急性肾衰竭(ARF)患者的临床疗效。方法选取50例NS并发ARF患者作为研究对象,随机分为研究组和对照组,每组25例。在常规对症治疗基础上,对照组给予呋塞米联合泼尼松治疗,研究组予以托拉塞米联合甲基泼尼松龙治疗。比较两组患者治疗前后血清尿素氮(BUN)、血清肌酐(SCr)、TC、ALB水平、24 h尿量及两组患者的治疗总有效率。结果治疗后,两组患者的BUN、SCr、TC水平均低于治疗前,且研究组患者低于对照组;两组患者ALB水平及24 h尿量均高于或多于治疗前,且研究组高于或多于对照组(均P<0.05)。研究组患者的治疗总有效率高于对照组(P<0.05);两组复发率比较差异无统计学意义(P>0.05)。结论托拉塞米联合甲基泼尼松龙治疗NS并发ARF患者,可以更有效地改善机体肾功能,提高利尿效果,临床效果更好。Objective To explore the clinical effect of torasemide combined with methylprednisolone on nephrotic syndrome（NS） complicated with acute renal failure（ARF）. Methods Fifty NS patients complicated with ARF were enrolled and were randomly divided into study group and control group,with 25 cases in each group.Based on the conventional treatment,the control group received furosemide combined with prednisone,while the study group was given torasemide combined with methylprednisolone.The levels of blood urea acid（BUN）,serum creatinine（SCr）,TC,ALB and 24 h urine output were compared between two groups before and after treatment,and the total effective rate was also compared between the two groups. Results After treatment,the levels of BUN,SCr and TC decreased while the levels of ALB and 24 h urine output increased in the two groups in contrast with those before treatment,and the levels of BUN,SCr and TC were lower while the levels of ALB and 24 h urine output were higher in the study group than those in the control group（all P 〈0.05）.The total effective rate in the study group was higher than that in the control group（ P 〈0.05）,and there was no significant difference in the recurrence rate between the two groups（ P 〉0.05）. Conclusion Torasemide combined with methylprednisolone can effectively improve renal function and promote diuresis effect,achieving a better the clinical efficacy for NS complicated with ARF.
