摘  要:目的通过观察难愈合性创面模型大鼠Ⅰ型胶原蛋白和Ⅲ型胶原蛋白表达情况,探讨皮肤创面无机诱导活性敷料促进难愈合性创面模型大鼠创面愈合的可能机制。方法将18只大鼠随机分为3组:即非难愈合性创面的对照组、模型组和实验组,每组6只。对照组仅做皮肤全切手术;其余两组经皮肤全切后给予醋酸氢化可的松皮下注射,制备难愈合性创面模型。造模后第4天,实验组大鼠创面每日给予皮肤创面无机诱导活性敷料软膏干预;其余2组大鼠创面每日进行常规换药处理。干预后第7天取材,计算各组大鼠创面愈合率。每组大鼠的一侧创面用作免疫组化检测Ⅰ型胶原蛋白和Ⅲ型胶原蛋白表达情况,另外一侧用于Western blot和qPCR检测Ⅰ型胶原蛋白、Ⅲ型胶原蛋白和mRNA表达量。结果对照组创面愈合率为(83.2±8.9)%,模型组创面愈合率为(68.7±7.3)%,实验组创面愈合率为(77.6±9.5)%,实验组创面愈合率显著高于模型组,差异具有统计学意义(P <0.05);免疫组化显示实验组的Ⅰ型胶原蛋白和Ⅲ型胶原蛋白的平均光密度分别为0.067±0.003和0.299±0.001,显著多于模型组的0.039±0.004和0.229±0.006,差异具有统计学意义(P=0.05);Western blot结果显示实验组的Ⅰ型胶原蛋白和Ⅲ型胶原蛋白的蛋白表达量分别为0.601±0.005和0.587±0.016,高于模型组的0.225±0.015和0.307±0.007,差异具有统计学意义(P<0.05);qPCR结果显示实验组的Ⅰ型胶原蛋白mRNA和Ⅲ型胶原蛋白mRNA表达量分别为5.81±0.87和3.54±0.94,显著多于模型组的1.77±0.42和1.98±0.84,差异具有统计学意义(P <0.05)。结论皮肤创面无机诱导活性敷料对难愈合性创面模型大鼠的创面愈合有促进作用,其作用机制可能与其促进创面局部Ⅰ型胶原蛋白和Ⅲ型胶原蛋白的表达有关。Objective To explore the possible mechanism of Dermlin on rats with refractory healing wound by observing the expression of Collagen Ⅰ and Collagen Ⅲ on rats with refractory healing wound. Methods Eighteen rats were divided into 3 groups randomly involving control group, model group and experiment group. Rats in control group underwent the total skin resection surgery only, and rats in the other two groups were used to establish the model of refractory healing wound after total skin resection surgery. Four days after surgery, rats in experiment group were administered Dermlin ointment on the wound every day, and wound dressing was applied on rats in the other groups every day. All rats were sacrificed on the fourteenth day after surgery and immunohistochemistry was used to detect expression of Collagen Ⅰ and Collagen Ⅲ , and Western blot was used to detect expression of CollagenⅠ protein and CollagenⅢprotein and qPCR was used to detect expression of Collagen Ⅰ mRNA and Collagen Ⅲ mRNA. Results The wound-healing rate in the control group was (83.2±8.9)%, the wound healing rate in the model group was (68.7±7.3)%, and the wound-healing rate was (77.6±9.5)% in the experimental group. The wound-healing rate in experiment group was significantly higher than that in the model group (P〈0.05). Immunohistochemistry shows that expressions of Collagen Ⅰ and Collagen Ⅲ in experiment group were more than those in model group with a statistic significance (P=0.05). Western blot shows that expression of CollagenⅠprotein and CollagenⅢprotein in experiment group were more than those in model group with a statistic significance (P〈0.05); qPCR shows that expression of mRNA of CollagenⅠ and CollagenⅢ in experiment group were more than those in model group with a statistic significance (P〈0.05). Conclusion Dermlin can promote the refractory wound healing, which may be associated with the mechanism that Dermlin up-regulates the expression of CollagenⅠ and CollagenⅢ .
