摘　　要：目的研究补肾培元胶囊对慢性阻塞性肺疾病肾阳虚证患者炎症因子及热休克蛋白水平的影响。方法选择2017年1~12月于广东省中西医结合医院呼吸内科门诊确诊并治疗的慢性阻塞性肺疾病稳定期60例为研究对象,根据随机数字表法分为治疗组和对照组,每组30例。对照组采用噻托溴铵粉吸入剂维持治疗,治疗组则联合补肾培元胶囊口服治疗。观察两组治疗效果,并采用酶联免疫吸附测定(ELISA)检测血清中肿瘤坏死因子-α(TNF-α)、白介素-4(IL-4)和白介素-8(IL-8)水平,采用双抗夹心酶免法测定血清中热休克蛋白70(HSP70)的表达水平。结果治疗组临床总有效率(93.3%)明显高于对照组(76.7%)(P<0.05)。两组患者治疗后TNF-α、IL-4和IL-8的表达均显著低于治疗前(P<0.05),但治疗组炎症因子表达水平下降更为明显(P<0.05);治疗组HSP70水平与治疗前比较有显著上升(P<0.05),而对照组HSP70无明显改善(P>0.05)。结论在常规治疗的基础上联用补肾培元胶囊治疗慢性阻塞性肺疾病肾阳虚证患者临床效果显著,可明显减轻患者炎性反应,同时提高热休克蛋白表达水平,从而对细胞起一定的保护作用。Objective To study the effect of Bushen Peiyuan Capsules on the levels of inflammatory factors and heat shock protein in chronic obstructive pulmonary disease patients with syndrome of deficiency of kidney yang. Methods Sixty patients with stable chronic obstructive pulmonary disease diagnosed and treated in Guangdong Integrated Traditional Chinese and Western Medicine Hospital from January to December 2017 were selected as research objects. The patients were divided into two groups according to the random number table, with 30 cases in each group. The control group was given Tiotropium Bromide Powder Inhalation to maintain treatment, while the treatment group was added with Bushen Peiyuan Capsules orally. The therapeutic effect between the two groups was observed. Enzyme-linked immunosorbent assay（ELISA） was used to determine the serum levels of tumor necrosis factor-α（TNF-α）, interleukin-4（IL-4） and interleukin-8（IL-8）. The expression levels of serum heat shock protein-70（HSP70） were determined by double-antibody sandwich enzyme immunoassay. Results The total effective rate in the treatment group（93.3%） was significantly higher than that in the control group（76.7%）（P〈0.05）. After treatment, the expression levels of TNF-α,IL-4 and IL-8 in the two groups were significantly lower than those before treatment（P〈0.05）, but the expression levels of inflammatory factors in the treatment group were decreased more significantly（ P〈0.05）. After treatment,the HSP70 level in the treatment group was significantly higher than that before treatment（P〈0.05）, while the control group had no significant improvement（P〉0.05）.Conclusion On basis of conventional treatment,Bushen Peiyuan Capsules in the treatment of chronic obstructive pulmonary disease patients with syndrome of deficiency of kidney yang has a significant clinical effect, which can reduce the inflammatory response, improve the expression of heat shock protein, thus protecting the cells from being damaged.
