摘 要:目的分析粘液型铜绿假单胞菌的临床分布及耐药性,为临床合理选择抗生素提供依据。方法对2014-2016年本院分离的粘液型铜绿假单胞菌临床分布及耐药性进行回顾分析。采用K-B纸片扩散法进行药敏试验。结果三年临床标本共分离出铜绿假单胞菌525株,其中粘液型铜绿假单胞菌84株,占16%;粘液型铜绿假单胞菌主要分离自临床呼吸内科;标本类型以痰为主,分离50株占59. 52%。其次为烧伤科分离的分泌物14株占16. 67%。粘液型铜绿假单胞菌对亚胺培南、美罗培南、阿米卡星敏感率为100%;对派拉西林/他唑巴坦敏感率为〉98%;对头孢他啶、头孢吡肟、环丙沙星、左氧氟沙星的敏感率为95%-97%;对庆大霉素、氨曲南的敏感率为90%。未发现多药耐药菌株。结论粘液型铜绿假单胞菌对临床常用的10种抗生素虽有较高的敏感性,但临床医生应根据药敏结果并考虑细菌生物膜影响因素而合理选择抗生素。Objective To analyze the clinical distribution and drug resistance of mucoid Pseudomonas aeruginosa so as to provide evidence for clinical rational selection of antibiotics. Methods A retrospective analysis was made to the clinical distribution and drug resistance of mucoid Pseudomonas aeruginosa isolated during the period from 2014 to2016; K-B paper disk diffusion was applied in drug sensitivity test. Results 525 strains of Pseudomonas aeruginosa were isolated from clinical specimens in the 3 years including 84 strains of Pseudomonas aeruginosa( 16%),mainly isolated from the sputum specimens from department of respiratory medicine,the isolation rate was 59. 52%; 14 strains was isolated from secretion specimens from burn department,taking up 16. 67%; the sensitivity of mucoid Pseudomonas aeruginosa to imipenem,meropenem and amikacin was 100% while that to piperacillin/tazobactam was 98%;the sensitivity of the isolated mucoid Pseudomonas aeruginosa to cefazolin,cefepime,ciprofloxacin and levofloxacin ranged from 95% to 97% while that to gentamicin and aztreonam kept 90%; no strains with multi-drug resistance were detected out.Conclusions Mucoid Pseudomonas aeruginosa is of higher sensitivity to 10 antibiotics commonly used in clinical and doctors need to make rational selection of antibiotics in consideration of drug sensitivity and the influencing factors of bacterial biofilm.
